Neuromonitoring of patients with traumatic brain injury from nursing perspective

Abstract:

Cíl: Cílem práce bylo zjistit, jaká je úroveň znalostí sester v problematice neuromonitoringu v ošetřovatelské péči o pacienta s traumatickým poraněním mozku. Dílčí cíle mapovaly úroveň znalostí sester v souvislosti s délkou praxe ve zdravotnictví, délkou praxe na současném pracovišti, vzděláním, absolvováním specializačního studia a osobní zkušeností s péčí o tyto pacienty. Metodika: Výzkumný soubor byl sestaven ze 147 sester pracujících na dvou Neurochirurgických jednotkách intenzivní péče a čtyřech odděleních Anesteziologicko-resuscitační kliniky Fakultní nemocnice Ostrava, dále na dvou odděleních Anesteziologicko-resuscitační kliniky Fakultní nemocnice Motol a jedné Neurochirurgické jednotce intenzivní péče v téže nemocnici a sester pracujících na Anesteziologicko-resuscitační klinice a Neurochirurgické jednotce intenzivní péče Fakultní nemocnice Olomouc. Výzkum v těchto nemocnicích probíhal od 1. 10. do 31. 12. 2013. Pro sběr dat byl použit nestandardizovaný dotazník sestavený výzkumníkem. Data byla zpracována statistikem. Statistické testy byly hodnoceny na hladině významnosti 0,05. Výsledky: Z hlediska celkového hodnocení mělo pouze 11,6 % respondentů znalosti na úrovni výborné, to znamená, že z počtu 21 otázek zodpověděli chybně maximálně 3 položky v dotazníku. Nejvíce 6 chybně zodpověděných otázek měla největší část respondentů (44,9 %), velká část respondentů však měla chybně 7 a více položek (43,5 %). Výzkum jednoznačně prokázal, že úroveň znalostí se liší na jednotlivých pracovištích, vyšší úroveň znalostí prokázaly sestry pracující na specializovaných neurochirurgických JIP. Dále byl zjištěn statisticky významný rozdíl v úrovni znalostí sester v závislosti na délce praxe na současném pracovišti, vzdělání a osobní zkušenosti v péči o pacienty s kraniocerebrálním poraněním. Naopak absolvování specializačního studia a délka praxe ve zdravotnictví jsou faktory, které na úroveň znalostí sester ve zkoumané problematice vliv nemají. Závěr: Vzhledem k tomu, že téměř polovina respondentů měla chybně zodpovězeno 7 a více dotazníkových položek, můžeme konstatovat, že ve znalostech sester v problematice neuromonitoringu existují určité rezervy. Přes 90 % respondentů se vyjádřilo, že své znalosti o monitorovacích technikách nepovažují za dostačující. K zajištění vysoce kvalitní péče o pacienty s traumatickým poraněním mozku je nutné nadále zvyšovat rozsah vědomostí a motivovat sestry k celoživotnímu vzdělávání.

Abstract:

Aim: The main aim of this thesis was to determine the level of knowledge in neuromonitoring of patients with traumatic brain injury from nursing perspective. Next goals focused on the level of knowledge of nurses in connection with the length of practice in healthcare, their length of practice at current workplace, their education, in connection with a specialized training and personal experience with nursing care of these patients. Methodology: The research sample was composed of 147 nurses working at two Neurosurgical Intensive Care Units (ICUs) and four Anesthesiology departments of the University Hospital in Ostrava, at Neurosurgical ICU and two Anesthesiology departments of the University Hospital in Motol (Prague) and finally the Neurosurgical ICU and Anesthesiology department of the University Hospital in Olomouc. The research data in these hospitals was collected between 1st October and 31st December 2013. The results of a non-standardized questionnaire assembled by the researcher was then analyzed by a statistician. Statistic tests were evaluated at a significance level of 0,05. Results: In terms of general evaluation of the results, only 11,6 % of respondents had excellent level of knowledge with at most 3 wrong answers out of 21 in the questionnaire. A major group of the research sample (44,9 %) answered wrongly 4 to 6 times, and a similarly noticable 43,5 % of the group answered incorrectly 7 or more times. The research clearly showed significant differences among the departments and nurses working at specialized Neurosurgical ICUs reached a higher score in general. A relevant variance of the nurses? level of knowledge was demonstrated in connection with their length of work experience at the current workplace as well as their education and personal experience with patients with a craniocerebral injury. Specialized training and the total length of health care experience proved to have no significant effect on the result. Conclusion: With almost half of the respondents answering wrongly 7 or more times, the study showed an apparent space for improvement. Over 90 % of the respondents stated they considered their knowledge insufficient themselves. To ensure high quality care for patients with traumatic brain injury it is vital to improve the level of knowledge and to motivate nurses to lifelong education.
