Physiotherapy after LCA reconstruction and instrumentation evaluation the effects of treatment

Abstract:

Cíl: Cílem mé bakalářské je přispět k řešení problematiky u pacienta s poraněním LCA. Metodika: Vybraný proband absolvoval 2 artroskopické operace, přičemž při první operaci byla provedena resekce obou zadních rohů menisků a při druhé operaci plastika předního zkříženého vazu patelárním štěpem. Před artroskopickou operací jsem u mého probanda odebral anamnézu a provedl jsem vstupní kineziologický rozbor a 3D kinematickou analýzu pohybu pro objektivizaci efektu terapie s následnou fyzioterapií. Po operaci jsem provedl kontrolní měření s následným stanovením krátkodobého fyzioterapeutického plánu. Ke zlepšení celkového stavu pacienta byly provedeny myofasciální techniky, aktivní a pasivní pohyb, odporová cvičení a metody na neurofyziologickém podkladě. Výsledky: U vybraného probanda byla vlivem operace výrazně snížena svalová síla PDK a značně omezen rozsah pohybu v kolenním kloubu. Vlivem ucelené rehabilitace došlo k obnovení svalové síly a dosažení plné hybnosti. Dále bylo zřejmé celkové zlepšení postury. Zmírnil se horní zkřížený syndrom a předsunuté držení těla již není tak výrazné. Také můžeme pozorovat zlepšení v oblasti pohybových stereotypů DK.

Abstract:

Objective: The aim of this work is contribution to solution of issues for patient with ACL injury. Methodology: Selected patient had two arthroscopic surgery. During the first surgery were both caudal corners of meniscis resected. During the second surgery, the plastic surgery of ACL was made with the patella implant. Before arthroscopic surgery, I took the case-history and input kinesiology analysis and 3D kinematic motion analysis were made. The device evaluation was made for the objectification of the effect of therapy followed by physiotherapy. After the operation I made control measurements and I specified short-term physiotherapy plan. To improve the overall condition of the patient, myofascial techniques, active and passive movement, resistance exercises and methods based on the neurophysiology were made. Results: Because of the surgery, patient had reduced muscle strength of right lower extremity. There was also significantly limited range of motion in the knee joint. Due to the comprehensive rehabilitation was muscle strength restored and full join movement was reached again .We can seen that upper crossed syndrome and forward posture are not so significant anymore. There is also obvious overall improvement in posture and in the movement stereotypes of both lower extremities.
