Molecular Diagnostics of SPINK1 and PRSS1 Genes in Patients with Hereditary Pancreatitis

Abstract:

Hereditární pankreatitida (HP) je autosomálně dominantní onemocnění, kdy opakující se záchvaty akutní pankreatitidy (AP) postupují do chronické pankreatitidy (CP). Symptomy se obvykle objevují již v dětství, či dospívání a časem se může dostavit exokrinní nebo endokrinní insuficience. Hereditarní pankreatitida souvisí s mutacemi v genu pro kationický trypsinogen (PRSS1) a v genu pro inhibitor serinové proteázy Kazal typ 1 (SPINK1). Mezi nejčastěji vyskytující se mutace u pacientů s hereditární pankreatitidou patří mutace p.R122H a p.N29I v genu PRSS1 a mutace p.N34S v genu SPINK1. Práce obsahuje výsledky sekvenování genů PRSS1 a SPINK1 u skupiny 5 pacientů s příznaky hereditární pankreatitidy. Ve vyšetřované skupině pacientů byly dvě ženy a tři muži a jejich průměrný věk byl 27 let (3-50 let). Na genetické došetření byli pacienti posláni gastroenterologem. DNA pacientů byla již dříve vyšetřována na základní spektrum nejčastějších mutací v genech PRSS1 a SPINK1. Genetickým došetřením jsme potvrdili přítomnost již dříve nalezené mutace p.N34S v genu SPINK1 v heterozygotním stavu. Cílem této bakalářské práce bylo došetření pacientů a rozšířit genetické testování o skríning mutací v těchto výše uvedených genech.

Abstract:

Hereditary pancreatitis is an autosomal dominant disease, which results in recurrent attacks of acute pancreatitis, progressing to chronic pancreatitis. Symptomps start to appear already in a childhood or in a puberty and with time it can turn up in exocrine or endocrine insuficiency. HP is connected with mutations in the cationic trypsinogen gene (PRSS1) and with mutations in the serine protease inhibitor Kazal type 1 (SPINK1). The most frequently mutations in the patients with hereditary pancreatitis are p.R122H and p.N29I in the PRSS1 gene and p.N34S in the SPINK1 gene. This work contains the results from the sequencing analysis of the PRSS1 and SPINK1 genes in the group of patients with symptoms of hereditary pancreatits (n=5). In our investigated group were 2 women and 3 men and their average age was 27 years (3-50 years). For the genetic testing patients were sent by the gastroenterologist. DNA of the patients was previously tested for basic spektrum of the most frequent mutations in the PRSS1 and SPINK1 genes. We confirmed a presence of the p.N34S mutation previously found by the genetic testing in the SPINK1 gene in the heterozygous state. The aim of this work was an investigate mentioned group of the patients and to extend a genetic testing for mutation´s screening in the entire PRSS1and SPINK1 genes.
