Subjective experience of disease in patients after spinal cord injury

Abstract:

Cíl: Získání autentických informací o prožívání nemoci u pacientů po poranění míchy. Metodika: Pro dosažení stanoveného cíle byla zvolena metoda kvalitativního výzkumu prostřednictvím polostrukturovaných rozhovorů, metodou interpretativní fenomenologické analýzy. Hloubkové rozhovory byly vedeny individuálně s jednotlivými participanty (n=6). Audionahrávky jednotlivých rozhovorů byly za využití metody doslovné transkripce přepsány do textové podoby. Prostřednictvím obsahové analýzy jednotlivých rozhovorů a metody otevřeného kódování a barvení textu byly vygenerovány jednotlivé kategorie a podkategorie. Na základě těchto kategorií byly vyhodnoceny výsledky. 1 Okolnosti vzniku poranění. 2. Akutní fáze poranění. 3. Postakutní fáze poranění. 4. Hodnocení ošetřovatelské péče. 5. Chronická fáze poranění. 6. Prožívání fyzických změn. 7. Prožívání psychických změn. 8. Prožívání sociálních změn. 9. Vnímání budoucnosti, motto. Výsledky: Poranění míchy se hluboce promítlo do života participantů. Zpočátku byla intenzivně zasažena fyzická a psychická oblast. Zúčastnění čelili negativním pocitům a emocím v souvislosti se sdělením diagnózy, se získaným deficitem, závislosti na okolí a nutností opětovného nácviku denních činností. V subakutní a chronické fázi byla rovněž zasažena sociální oblast. Zúčastnění popisují četné obtíže v procesu sociální integrace. Rovněž uvádějí negativní změny v sociálních rolích, změny v práci a v rodině. Společným znakem, který se opakuje u všech participantů, je naděje na uzdravení. Závěr: Závěrem lze říci, že se výpovědi zúčastněných ve velké míře shodovaly a poranění míchy představuje stigmatizující událost, který má negativní vliv na fyzickou, psychickou i sociální oblast. Získané výsledky by mohly pomoci porozumět a pochopit prožívání pacientů po poranění míchy. Výsledky mohou přispět k vyšší empatii ošetřovatelského personálu a podpoře pacientů se spinálním poraněním.

Abstract:

Aim: To acquire authentic information about experience of patients with spinal cord injury. Methodology: In order to achieve the set aim, the method of qualitative survey using semi-structured interviews by means of interpretative phenomenological analysis was selected. In-depth interviews were held individually with the respective participants (n=6). Audio recordings of the individual interviews were transcribed into text form using the method of literal transcription. Individual categories and subcategories were generated using the content analysis of the individual interviews and the method of open coding and text marking. The results were assessed based on these categories. 1 Circumstances of injury occurrence. 2. Acute phase of injury. 3. Post-acute phase of injury. 4. Rating of nursing care. 5. Chronic phase of injury. 6. Experience of physical changes. 7. Experience of psychic changes. 8. Experience of social changes. 9. Perception of future, motto. Results: Spinal cord injury had a deep impact on the participants´ lives. At first, there was an intensive impact on physical and psychic spheres. The participants faced negative feelings and emotions related to diagnosis announcement, with acquired deficiency, dependency on other people and the need of re-exercising the daily activities. Social sphere was also impacted in the post-acute and the chronic phase. The participants also reported difficulty in social integration process. They also reported negative changes in social roles, changes at work and in family. Prospect of recovery is the common feature present with all the participants. Conclusion: It can be concluded that the participants´ reports coincided in a large extent and that spinal cord injury represents a stigmatizing event having negative influence on physical, psychic and social spheres. The acquired results could help to understand the experience of patients with spinal cord injury. These results can also contribute to a higher empathy of the nursing care personal and to the support of patients with spinal cord injury.
