Physiotherapy in total knee arthroplasty

Abstract:

Cíl práce: Cílem této práce je přispět k řešení problematiky fyzioterapie u totální aloplastiky kolenního kloubu a seznámit veřejnost s průběhem rehabilitace v předoperační a pooperační fázi a s využitím jednotlivých metod a konceptů během terapie. Metodika: Zpracoval jsem kazuistiku 35 letého pacienta po implantaci totální endoprotézy kolenního kloubu vlevo. Fyzioterapeutická intervence probíhala v období 8 týdnů, je rozdělena na období předoperační, hospitalizační a ambulantní. Před operací jsem provedl vstupní kineziologický rozbor zaměřený na anamnézu, vyšetření aspekcí a palpací, antropometrii, goniometrii, vyšetření svalové síly, zkrácených struktur, kloubní vůle, pohybových stereotypů a chůze. Po operaci jsem provedl kontrolní vyšetření zaměřené na status localis operované DK. Na základě těchto vyšetření jsem stanovil krátkodobý fyzioterapeutický plán s využitím konkrétních metod a postupů - techniky měkkých tkání, cévní a dechová gymnastika, míčková facilitace, aktivní a pasivní cvičení, cvičení v uzavřeném kinematickém řetězci, cvičení s pomůckami (theraband, overball), postizometrická relaxace, postfacilitační strečink, metoda PNF a ACT či senzomotorická stimulace. Uvádím zde celkem 13 terapií, 1 terapie ve fázi předoperační, 10 terapií během hospitalizační fáze a 2 terapie ve fázi ambulantní. Délky jednotlivých terapií se pohybovaly kolem 45 - 60 minut. 1., 4., 11. den a 3. týden po operaci jsem provedl kontrolní vyšetření se zaměřením na pohyblivost a svalovou sílu dolních končetin. Po ukončení hospitalizační i ambulantní fyzioterapie jsem provedl výstupní kineziologický rozbor a stanovil dlouhodobý fyzioterapeutický plán. Pro získání co nejlepších výsledků jsem provedl ještě kontrolní kineziologický rozbor 4 měsíce od operace a porovnal výsledky získaných měření před fyzioterapeutickou intervencí, během intervence a po intervenci. Výsledky: Díky operaci došlo k vyrovnání rozdílu mezi délkami končetin a zmenšení patologické varosity operované končetiny. Došlo ke zlepšení hybnosti a svalové síly oproti stavu před operací. Jizva je dobře zahojená a posunlivá, stejně tak měkké tkáně v oblasti levé dolní končetiny. Došlo k zlepšení pohyblivosti caput fibulae a patelly s odstraněním patologických bariér. Přetrvává problém s některými pohybovými stereotypy a s otokem v oblasti kolene, který lehce omezuje hybnost, proto doporučuji pokračovat v ambulantní rehabilitaci. Závěr: Totální endoprotéza kolenního kloubu je dnes již rutinní operace. Napomáhá tomu rychlý rozvoj medicíny a rehabilitačních možností. Mladý věk mého pacienta se ukázal být výhodou, pacient skvěle spolupracoval, což se pozitivně projevilo na výsledcích. Dobře odvedená rehabilitace má nezastupitelný vliv u problematiky totálních endoprotéz. Pokud se vše provede bez problémů, pacient se cítí po operaci mnohem lépe, dochází k eliminaci bolesti a špatných pohybových stereotypů a k návratu do běžného života, zpravidla s lepší kvalitou než před operací.

Abstract:

Aim of the thesis: The aim of this thesis is to contribute to the issue of physiotherapy after total alloplasty of the knee joint and to introduce the process of the rehabilitation in the pre-operational and post-operational phase with the use of different methods and concepts to the public. Methodology: I have processed a case report on a thirty-five-year-old patient, who had a total knee endoprosthesis implanted into his left knee. Physiotherapeutic intervention was carried out during the course eight weeks and it is divided into three phases - pre-operational, inpatient and outpatient. I carried out an entry kinesiological analysis prior to the operation focused on anamnesis, aspection and palpation examination, anthropometry, goniometry, muscle strength, shortened structures, joint play, movement stereotypes and gait. Then I carried out an examination after the surgery focused on status localis of the operated leg and created a short-term physiotherapeutic plan based on the results, using specific methods and procedures: soft tissue mobilization, circulatory and breathing exercises, foam ball facilitation method, active and passive exercises, closed kinematic chain exercises, exercises with aides (theraband, overball), post isometric relaxation (PIR), post facilitation stretching (PFS), PNF and ACT methods and sensorimotor stimulation. I have outlined a total of thirteen therapies, one in the pre-operational, ten in the inpatient and two in the outpatient phases. The length of each individual therapy spanned around 45 to 60 minutes. I carried out an inspection on the 1st, 4th and 11th day after the surgery, focusing on the flexibility and muscular strength of the lower extremities. Upon the conclusion of the patient's outpatient treatment, I carried out a final kinesiological analysis, creating a long-term rehabilitation plan based on the results. For the purposes of accomplishing the best results possible, I carried out another kinesiological analysis four months after the surgery and compared the results with data recorded before physiotherapeutic intervention, during the intervention and after the intervention. Result: The lengths of both extremities were evened and the pathological varosity of the operated extremity was reduced thanks to the operation. The flexibility and the muscular strength of the limb was improved compared to its state prior to surgery. The scar is well-healed and movable, as is the soft tissue in the lower limb. With the pathological barriers now removed, the flexibility of the caput fibulae and the patella has improved. Some problems remain, specifically with certain movement stereotypes and swelling in the knee area, which slightly impedes movement, which is why I recommend the continuation of outpatient therapy. Conclusion: Today, aided by the fast progress of medical science and rehabilitation options, total knee-joint replacement is a routine surgery. The young age of my patient turned out to be an advantage - the patient cooperated well, which positively affected the results. A well carried out rehabilitation has an irreplaceable effect in total joint replacements. If everything goes without any problems, the patient feels much better after the surgery, pain and negative movement stereotypes are eliminated and the patient can return to normal life with a much better quality of health.
