Institutional And Financial Determinants Of Public Health Insurance System Operating In The Czech Republic And Their Influence On Deficits

Abstract:

Disertační práce je zaměřená na problematiku finanční bilance systému veřejného zdravotního pojištění v České republice. Cílem práce je identifikovat na pozadí analýzy vývoje právního a institucionálního prostředí faktory, které se podílely na vzniku deficitu a kumulaci dluhu systému veřejného zdravotního pojištění v ČR a analyzovat jejich váhu. Výzkumnou otázkou je ověření předpokladu, zda za hlavní příčinu vzniku a kumulace deficitu lze považovat bezprostřední kroky vlády v oblasti úhrad za zdravotní péči. Práce je rozdělena do pěti kapitol. První kapitola se zabývá teoretickými východisky k financování zdravotní péče a deficitu. Ve druhé kapitole je popsán vývoj v oblasti veřejného zdravotního pojištění v ČR od roku 1991 včetně analýzy relevantní legislativy. Třetí kapitola je věnována úhradovým mechanismům používaným ve sledovaném období v ČR. Čtvrtá kapitola obsahuje analýzu hospodaření zdravotních pojišťoven v letech 1998-2008 s použitím regresní analýzy. Pátá, závěrečná kapitola představuje závěry analýz a identifikaci faktorů ovlivňujících jednotlivé proměnné finanční bilance systému veřejného zdravotního pojištění.

Abstract:

The thesis is focused on the issue of financial balance of public health insurance system in the Czech Republic. The objective is, within the context of the legal and institutional background, to identify the factors contributing to the existence of the deficit and cumulative debt of public health insurance system and to analyze their weight. The research question is to verify the assumption that the main cause of deficit emergence is the interventions of the government within the field of reimbursements regulations. The thesis is divided into five chapters. The fist one deals with theoretical background about health care financing and deficits. Second chapter describes public health insurance development since 1991, including the legislation analysis. Third chapter is dedicated to the means of reimbursements used in Czech Republic. Another chapter contains the analysis of public health insurance companies´ economy, using the method of regression analysis. In final chapter the conclusions of analyses and identification of factors influencing the variables determining the financial balance of health insurance system are introduced.
