The use of stem cells in treatment of articular cartilage defect

Abstract:

Úvod Poranění kloubní chrupavky má velice malý potenciál hojení, proto i léčba defektů kloubní chrupavky je všeobecným problémem. Současné možnosti léčby defektů kloubní chrupavky vedou pouze k nedokonalému zhojení neplnohodnotnou tkání. Experimentální práce se zaměřuje na hledání nových možností, které by se přiblížily ideálnímu způsobu terapie, tj. kompletní regeneraci plnohodnotnou hyalinní chrupavkou se zachováním stejných funkčních vlastností, využitím mesenchymových kmenových buněk a jejich transplantací do iatrogenně poraněné kloubní chrupavky. Materiál a metodika Experiment byl prováděn na modelu miniaturního prasete. Vytvořili jsme vlastní metodiku celého experimentu. Soubor 30 miniaturních prasat byl po 10 pokusných zvířatech rozdělen do 3 experimentálních skupin. Skupině A byl vytvořen vrtáním defekt průměru 6×12 mm do oblasti zátěžové plochy kloubní chrupavky distálního femoru a do něj byl transplantován kolagen/chitosanový nosič (skafold) osazený MSCs. Skupině B byl vytvořen defekt stejného rozměru ve stejné oblasti, byl však osazen pouze skafoldem bez kmenových buněk. Skupina C tvořila kontrolní skupinu, které byl vytvořen defekt pomocí kruhového skalpelu průměru 6 mm v oblasti zátěžové plochy kloubní chrupavky kolena a byl ošetřen metodou mikrofraktury. Celý soubor byl sledován po dobu 16 týdnů, po té byla všechna zvířata protokolárně utracena. Distální femory pokusných zvířat byly vypreparovány a podrobeny podrobnému makroskopickému i histologickému vyšetření. Výsledky Defekty kloubní chrupavky distálního femoru ve skupině A vykazovaly makroskopicky v 60 % hojení bez patologických projevů charakteru osteofytů, adhezí nebo trhlin. Defekty ve skupině B a C se nezhojily tímto způsobem ani v jednom případě. Výsledky histologického a histochemického vyšetření nově formované hylinní chrupaky byly klasifikovány modifikovaným O´Driscollovým skorem, skupina A 14.7±3.82 bodů, skupina B 5.3±2.88 bodů a skupina C 5.2±0.64. Diskuse Pro náš experiment jsme vytvořili vlastní druh nosiče, který je biokompatibilní, biodegradabilní, dostatečně porozní pro inkorporaci MSCs a snadno manipulovatelný, složen je z chitosanových nano a mikrovláken a kolagenu. Je otázkou, zda by bylo možné zlepšit výsledky hojení úpravou skafoldu, lepší retencí MSCs v oblasti poraněné chrupavky nebo množstvím MSCs ve skafoldu. Otázkou je i použití imunosuprese v průběhu experimentu. Závěr Transplantace allogenních MSCs kombinaci s vhodným skafoldem vede k prokazatelně lepšímu hojení defektu kloubní chrupavky v oblasti zátěžové plochy mediálního kondylu stehenní kosti na modelu miniaturního prasete ve srovnání s kontrolními skupinami.

Abstract:

Introduction Articular cartilage injuries exhibit low healing potential and the treatment of articular cartilage defects is therefore generally considered problematic. Current treatment options for articular cartilage defects only lead to imperfect healing resulting in the formation of inadequate tissue. The objective of our experimental study was to find new options that would approximate to the ideal treatment method, i.e. achieving complete regeneration with the formation of adequate hyaline cartilage while fully maintaining its functions, by means of mesenchymal stem cell transplantation into an iatrogenic defect of articular cartilage. Material and methods In our experiment miniature pig was used as a model. The experiment drew entirely on our own methodology. A group of 30 miniature pigs was divided into 3 experimental groups (10 pigs in each). In group A a defect of 6x12 mm was created by drilling in the weight-bearing section of articular cartilage of distal femur and a collagen- and chitosan-based scaffold seeded with MSCs was transplanted into the defect. In group B a defect of the same dimensions and in the same area was created but in this group a scaffold without stem cells was transplanted. Group C was a control group where a defect was created by a circular scalpel of 6 mm diameter in the weight-bearing section of articular cartilage of the knee and it was treated with microfracture. All experimental animals were monitored for 16 weeks and subsequently euthanized lege artis. Distal femora of the animals were extracted and thorough macroscopic and histological examination was performed. Results Macroscopic observation of articular cartilage defects in distal femur in group A revealed healing without pathological changes, such as osteophytes, adhesion or disruption, in 40% of cases. In group B and C the same degree of healing was observed only in one case (10%). The results of histological and histochemical examination of the newly-formed hyaline cartilage were classified using the modified O’Driscoll score: group A 14.7±3.82 points, group B 5.3±2.88 points and group C 5.2±0.64 points. Discussion Our experiment required the preparation of a specific scaffold type: a biocompatible and biodegradable scaffold with sufficient porosity enabling the incorporation of MSCs, easy to manipulate, composed of chitosan nano- and microfibers and collagen. Whether better healing results could be achieved by scaffold adjustment, improved retention of MSCs at the site of damaged cartilage or by a different amount of MSCs on the scaffold is open to discussion. The use of immunosuppression in the experiment is also worth considering. Conclusion The transplantation of allogeneic MSCs combined with a suitable scaffold leads to significantly improved healing of articular cartilage defects in the weight-bearing section of the medial condyle of femur in miniature pigs compared to control groups.
