The role of epidermal growth factor receptor (EGFR) in targeted therapy of solid tumors

Abstract:

Receptor pro epidermální růstový faktor (EGFR) je transmembránovým receptorem s tyrosinkinázovou aktivitou, který hraje důležitou roli v regulaci buněčného cyklu, proliferace, diferenciace a přežívání buněk. Trvalá aktivace EGFR a navazujících signálních drah může přispívat k maligní transformaci buňky a stimulovat procesy vedoucí k růst nádoru a jeho progresi, jako jsou migrace, invaze a angiogeneze. Proto EGFR představuje jeden z vhodných cílů v protinádorové léčbě. Dosud bylo vyvinuto několik EGFR inhibitorů (monoklonální protilátky či malé tyrosinkinázové inhibitory), které jsou široce používané v klinické praxi pro léčbu různých solidních nádorů: nemalobuněčný karcinom plic, karcinom tlustého střeva a konečníku, karcinom hlavy a krku. Cílem této bakalářské práce je shrnutí poznatků o EGFR a jeho signalizaci u solidních nádorů, dále o využití EGFR inhibitorů v cílené protinádorové léčbě. Zabývá se též aspekty selekce pacientů vhodných pro cílenou léčbu na základě analýzy molekulárních markerů na úrovni mutací či exprese genů/proteinů.

Abstract:

The epidermal growth factor receptor (EGFR) is a transmembrane receptor with tyrosin kinase activity, which plays an important role in cell-cycle regulation, proliferation, differentiation, and cell survival. Constitutive activation of EGFR and downstream pathways may contribute to malignant transformation of cells, and may stimulate processes leading to tumor growth and progression, including migration, invasion, and angiogenesis. Therefore EGFR represents one of the targets suitable for cancer treatment. Several EGFR inhibitors (monoclonal antibodies or small tyrosine kinase inhibitors) have been evolved so far and are broadly used in clinical practice for treatment of various solid tumors: non-small-cell lung cancer, colorectal cancer, head and neck cancer. The aim of this bachelor thesis is summary of the current knowledge about EGFR and downstream signalization in solid tumors, and about application of EGFR inhibitors as a targeted cancer therapy. Aspects of patient selection suitable for targeted therapy based on the analysis of molecular markers, including mutations, gene/protein expression, are broadly discussed.
