The assessment of depression and quality of life in elderly people in the community

Abstract:

Cíl: Cílem diplomované práce bylo zjistit výskyt deprese a posoudit kvalitu života seniorů žijících v institucionální péči a seniorů žijících v domácím prostředí. Zjišťoval se také rozdíl v kvalitě života s depresí a bez deprese a vliv sociodemografických údajů na výskyt deprese a kvalitu života. Metodika: Kvantitativní výzkum probíhal za pomocí dvou standardizovaných dotazníků. Deprese byla hodnocena pomocí Škály deprese pro geriatrické pacienty (GDS - 15) a kvalita života pomocí brief Older People Quality of Life questionnaire (OPQoL-brief). Do výzkumu bylo zařazeno celkem 117 seniorů, z toho 60 seniorů z institucionální péče a 57 seniorů z domácího prostředí. Výsledky: Bez deprese bylo v domácím prostředí celkem 32 seniorů (59,3 %) a v institucionální péči 22 seniorů (40,7 %). Mírná deprese se vyskytla u 22 seniorů (44,0 %) z domácího prostředí a u 28 seniorů (56,0 %) z institucionální péče. Těžká deprese se vyskytla častěji u seniorů v institucionální péči (10 seniorů; 76,9 %) oproti seniorům v domácím prostředí (3 senioři; 23,1 %). Nebyl však zjištěn statisticky významný rozdíl ve výskytu deprese mezi oběma skupinami. Průměrná hodnota kvality života u seniorů žijících v domácím prostředí byla 45,1 a v institucionální péči 44,3. Mezi oběma skupinami taktéž nebyl zjištěn statisticky významný rozdíl. Kvalita života seniorů byla výrazně ovlivněna při výskytu deprese, a to u obou sledovaných skupin. Na výskyt deprese u seniorů v domácím prostředí měl vliv věk, rodinný stav a subjektivní vnímání zdravotního stavu. V instituci nebyl zjištěn vliv žádného sociodemografického údaje. Na kvalitu života u seniorů v domácím prostředí měl vliv věk, rodinný stav, to, s kým senior žije, vzdělání a subjektivní vnímání zdravotního stavu. V instituci nebyl prokázán vliv žádného sociodemografického údaje. Závěr: Hodnocení deprese a kvality života je v seniorském věku velmi důležité. Deprese, která není včas podchycena a léčena výrazně ovlivní život jedince. Klíčová slova: deprese, kvalita života, senior, domácí prostředí, institucionální péče

Abstract:

Aim: The aim of the thesis was to determine the prevalence of depression and to assess the quality of life of seniors living in institutional care and seniors living at home. The difference in quality of life with and without depression and the effect of sociodemographic data on the prevalence of depression and quality of life were also investigated. Methodology: Quantitative research was conducted using two standardized questionnaires. Depression was assessed using the Geriatric Depression Scale (GDS - 15) and quality of life using the Older People Quality of Life questionnaire (OPQoL-brief). A total of 117 elderly people were included in the study, 60 elderly people from institutional care and 57 elderly people from home care. Results: A total of 32 seniors (59.3%) in the home setting and 22 seniors (40.7%) in institutional care were free of depression. Moderate depression was present in 22 seniors (44.0%) from the home setting and 28 seniors (56.0%) from institutional care. Severe depression occurred more frequently among seniors in institutional care (10 seniors; 76.9%) compared with seniors in the home setting (3 seniors; 23.1%). However, there was no statistically significant difference in the prevalence of depression between the two groups. The mean quality of life score for seniors living at home was 45.1 and for those in institutional care was 44.3. There was also no statistically significant difference between the two groups. The quality of life of the elderly was significantly affected by the occurrence of depression in both groups. Age, marital status and subjective perception of health status influenced the occurrence of depression in the elderly in the home environment. No influence of any sociodemographic data was found in the institution. Age, marital status, who the senior lives with, education and subjective perception of health status had an effect on quality of life in the homebound seniors. There was no influence of any sociodemographic data in the institution. Conclusion: Assessment of depression and quality of life is very important in the elderly. Depression that is not caught and treated early will significantly affect the life of the individual. Keywords: depression, quality of life, senior, home environment, institutional care
