Community psychiatry between caring, discipline and control

Abstract:

Disertační práce se věnuje proměnám praxe komunitní psychiatrie v souvislosti s reformou psychiatrické péče, nebo také péče o duševní zdraví, která probíhá v České republice. Reforma si vytkla za cíl destigmatizaci, deinstitucionalizaci a humanizaci psychiatrické péče. Tyto cíle se na poli komunitní psychiatrie realizují prostřednictvím reorganizace, jejímž pilířem je vznik sítě center duševního zdraví, nového typu sociálně-zdravotní služby. Na poli péče o osoby se závažnými psychickými potížemi tak v českém kontextu poprvé dochází k systematickému propojení zdravotní péče a sociální práce. Záměrem této práce je na základě etnografického výzkumu v jednom ze vznikajících center v letech 2017-2019 a genealogického studia politiky reformy zachytit moment, kdy se začíná realizovat změna na poli komunitní psychiatrie. Práce si klade otázku, jakým způsobem nově vznikající služby v implementační fázi reformy nabízejí svým uživatelkám a uživatelům prostor rozšiřující jejich možnosti porozumění a jednání v komplikované životní situaci spojené s psychickým strádáním. Politika reformy se vymezuje vůči tradičním psychiatrickým institucím, fungujícím na principu disciplíny a dohledu, a nabízí model založený spíše na mocenské technologii governmentality. Komunitní psychiatrie procházející reformou se nicméně jeví je pole, ve kterém se oba zmíněné způsoby organizace mocenských vztahů prolínají.

Abstract:

The dissertation focuses on changes in the practice of community psychiatry in the context of the reform of psychiatric care, or mental health care, that is taking place in the Czech Republic. The reform aims to destigmatise, deinstitutionalise, and humanise psychiatric care. In the field of community psychiatry, these objectives are being realised through a reorganisation whose pillar is the creation of a network of mental health centres, a new type of social-health service. This is the first time in the Czech context that health care and social work have been systematically linked in the field of care for people with serious mental health problems. Based on ethnographic research in one of the emerging centres in 2017-2019 and a genealogical study of the reform politics, the intention of this dissertation is to capture the moment when change in the field of community psychiatry begins to be realised. The dissertation asks how emerging services in the implementation phase of the reform offer their users a space to expand their possibilities for understanding and acting on the complex life situation associated with psychological distress. The reform policy sets it apart from traditional psychiatric institutions, operating on the principle of discipline and surveillance, and offers a model based rather on the power technology of governmentality. Community psychiatry undergoing the reform, however, appears to be a field in which both aforementioned arrangements of power relations intersect.
