Nursing process in the care of a patient with acute heart attack

Abstract:

Cílem bakalářské práce je vytvořit ošetřovatelský proces v péči o pacienta s akutním infarktem myokardu. Teoretická část práce je rozdělena na dvě kapitoly. V první kapitole je z medicínského hlediska popsána epidemiologie, etiologie, klinický obraz, diagnostika, léčba, komplikace a prevence vztahující se k akutnímu infarktu myokardu. Druhá kapitola teoretické části je zaměřena na role sestry a specifika ošetřovatelské péče. Průběh ošetřovatelské péče je popsán jak na jednotce intenzivní péče, kam je pacient obvykle odesílán ihned po revaskularizační terapii, tak na standardním oddělení, kam je obvykle po stabilizaci zdravotního stavu překládán. Podrobněji je popsána role sestry ve vyšetřovacích metodách v diagnostice onemocnění a je zmíněna důležitost edukace a potřeba psychické podpory pacienta. V empirické části byl vytvořen ošetřovatelský proces. U pacienta byla odebrána ošetřovatelská anamnéza po přijetí do nemocnice na jednotku intenzivní péče, podle které byly stanoveny ošetřovatelské diagnózy dle NANDA-I taxonomie II. K ošetřovatelským diagnózám byly vytvořeny ošetřovatelské intervence, které byly realizovány a následně zhodnoceny. Cíle ošetřovatelské péče vztahující se k ošetřovatelským diagnózám byly dle hodnocení v této práci částečně nebo plně splněny.

Abstract:

The aim of the bachelor´s thesis is to create a nursing process in the care of a patient with an acute heart attack. The theoretical part is divided into two chapters. The first chapter describes the epidemiology, etiology, symptoms, diagnosis, treatment, complications and prevention related to an acute heart attack from the medical point of view. The second chapter of the theoretical part focuses on the role of nurse and the specifics of nursing care. The course of nursing care is described both as practiced in the intensive care unit, where the patient is usually sent immediately after revascularization therapy, and in the standard ward, where the patient is usually transferred after their condition has been stabilized. The role of nurse regarding examination methods in the diagnosis of the disease is described in more detail and the importance of education and the needs for psychological support of the patient are mentioned. In the empirical part, a nursing process was developed. The nursing assessment of the patient was obtained after admission to the intensive care unit, afterwards nursing diagnosis according to NANDA-I Taxonomy II were determined. Nursing interventions for nursing diagnosis were developed, implemented and subsequently evaluated. According to the evaluation in the thesis, the goals of nursing care related to nursing diagnosis were partially or fully met.
