Assessment of toe brachial index validity in diabetic patients

Abstract:

Úvod: Toe brachial index (TBI) je alternativní metodou ankle brachial indexu (ABI). Obě metody jsou neinvazivní a používají se ke stanovení ischemické choroby dolních končetin (ICHDKK). Vyšetření ABI se doporučuje u všech pacientů starších 70 let, také u kouřících a zejména u diabeticky?ch pacientů starších 50 let, u ktery?ch je vy?skyt ICHDKK častější než u zbytku populace. TBI se doporučuje použít v případě, že arterie v oblasti kotníku nelze pomocí metody ABI změřit, např. z důvodu kalcifikace tepen, která se v této oblasti objevuje častěji než v palcích u nohou, kde se stanovuje TBI. Cíl: Cílem práce bylo vyhodnotit validitu metody TBI u diabetiků a zjistit, zda tato metoda přináší zlepšení oproti metodě ABI. Dílčími cíli bylo změřit a vyhodnotit TBI a ABI v souboru diabetiků, porovnat zjištěné vy?sledky TBI a ABI, vyhodnotit validitu TBI i ABI oproti metodě duplexní ultrasonografie (DUS), jako zvolenému vyšetřovacímu standardu pro stanovení ICHDKK v této práci. Posledním cílem bylo porovnat vy?sledné parametry validity TBI a ABI mezi sebou. Metodika: Byla provedena popisná statistika získany?ch anamnesticky?ch dat, u ktery?ch se sledovaly zejména antropometrické charakteristiky, rizikové faktory ICHDKK (diabetes, kouření, hypertenze), vy?skyt kardiovaskulárních onemocnění a bylo provedeno vyhodnocení subjektivních potíží dolních končetin. Dále bylo pomocí oscilometrické metody (ABI OSCI) a metody Dopplerovy (ABI DPP) změřeno ABI. Metoda ABI DPP se dopočítávala dvojím způsobem ABI DPP LAP a ABI DPP HAP. Hodnoty TBI byly zjištěny pomocí automatizované pletysmografické metody. Posledním vyšetřením byla duplexní ultrasonografie (DUS). Pomocí DUS bylo vyšetřeno 10 úseků krevního řečiště na každé dolní končetině a v každém úseku byly sledovány parametry: průměr artérie [mm], stenóza artérií [%], rychlost proudění v artériích [cm/s], tvar spektrální křivky, přítomnost, struktura a velikost AS plátů a případná kalcifikace. Vy?sledky: Byl zjištěn statisticky vy?znamny? rozdíl (p<0,05) jak mezi jednotlivy?mi metodami stanovení ABI, tak i mezi metodami ABI v porovnání s metodou TBI. V rámci hodnocení korelace mezi metodou ABI OSCI a ostatními metodami stanovení ABI, nejlépe, ačkoli velmi slabě, korelovala metoda ABI DPP LAP (r=0,26). S metodou TBI nejvíce, ačkoli také velmi slabě, korelovala metoda ABI DPP LAP (r=0,22). Při srovnáni s metodou DUS dopadla nejlépe metoda TBI, která ze všech zkoumany?ch metod identifikovala nejvíce končetin s ischemií, celkem 7 končetin z 8 zachyceny?ch pomocí DUS (senzitivita 88 %). Metoda TBI také vykazovala nejlepší specificitu (88 %), PPV (64 %), NPV (97 %), nejvyšší hodnotu LR+ 7,44 a nejnižší, tedy nejlepší, hodnotu LR- 0,14. Závěr: Metoda TBI vykazovala v našem souboru pacientů nejpřesnější vy?sledky v porovnání s metodou DUS oproti všem metodám stanovení ABI.

Abstract:

Introduction: Toe brachial index (TBI) is an alternative method of ankle brachial index (ABI). Both methods are non-invasive and are used to determine peripheral arterial disease (PAD). ABI is recommended in all patients older than 70 years, also in smokers and especially in diabetic patients older than 50 years, in whom the incidence of PAD is more frequent than in the rest of the population. It is recommended to use TBI if the ankle artery cannot be measured using the ABI method, e.g. due to arterial calcification, which occurs more frequently in this area than in the toes. Objective: The main objective of this thesis was to evaluate the validity of the TBI method in diabetics and to find out whether this method brings an improvement over the ABI method. The minor objectives were to measure and evaluate TBI and ABI in a group of diabetics, to compare the results of TBI and ABI and to evaluate the validity of TBI and ABI compared to the duplex ultrasonography (DUS) method as the chosen examination standard for determining PAD in this thesis. The last objective was to compare the resulting parameters of TBI and ABI validity with each other. Methods: Descriptive statistics of the obtained anamnestic data were performed, in which anthropometric characteristics, risk factors of PAD (diabetes, smoking, hypertension), the incidence of cardiovascular diseases were evaluated and an evaluation of subjective problems of the lower limbs was performed. Furthermore, the ABI was measured using the oscillometric method (ABI OSCI) and the Doppler method (ABI DPP). The ABI DPP method was calculated in two ways, ABI DPP LAP and ABI DPP HAP. TBI values were determined using an automated plethysmographic method. The last examination was duplex ultrasonography (DUS). Using DUS, 10 sections of the bloodstream on each lower limb were examined and the parameters in each section were monitored: artery diameter [mm], artery stenosis [%], artery flow rate [cm/s], spectral curve shape, presence, structure and size of AS plates and possible calcification. Results: A statistically significant difference (p <0.05) was found both between the individual methods of ABI determination and between the ABI methods in comparison with the TBI method. Within the evaluation of the correlation between the ABI OSCI method and other methods of ABI determination, the ABI DPP LAP method correlated the best, although very weakly (r = 0.26). The ABI DPP LAP method correlated most, although also very weakly (r = 0.22). When compared to the DUS method, the TBI method performed best, which of the most examined methods identified the most limbs with ischemia, a total of 7 limbs out of 8 captured by DUS (sensitivity 88%). The TBI method also showed the best specificity (88%), PPV (64%), NPV (97%), the highest value of LR + 7.44 and the lowest, i.e. the best, value of LR-0.14. Conclusion: The TBI method showed the most accurate results in our group of patients in comparison with the DUS method compared to all methods of ABI determination.
