Aggression in institutional care for people with intellectual disability

Abstract:

V diplomové práci jsem se zaměřila na potenciální zdroje, které mohou podněcovat vznik agrese v zařízeních ústavní péče, a to jednak na straně osob s mentálním postižením a jednak na straně zaměstnanců. Důraz byl přitom kladen na interakční kontext, ve kterém se agresivní chování v těchto zařízeních odehrává. Byl proveden empirický výzkum, který se zabýval způsoby, jakými zaměstnanci vysvětlují vznik agrese na jejich pracovišti. Dále se opíral o teoretická východiska atribučních procesů a vlivů, které mohou tyto procesy determinovat. Zvláštní pozornost byla věnována self-efficacy zaměstnanců. Výzkum vycházel z kvantitativního a kvalitativního přístupu, 55 zúčastněných respondentů písemně reagovalo na vyzývací fotografii, o jejímž obsahu vyprávěli příběh. Respondenti dále vyplňovali 2 posuzovací škály. První se zaměřovala na zjišťování atribučních faktorů dle Heiderovy typologie (lokalizace, stabilita a kontrola), druhá posuzovací škála pak určovala míru self-efficacy respondenta v oblasti zvládání problémového chování klienta. Dle výsledků nebyla zjištěna souvislost mezi mírou self-efficacy zaměstnanců a jednotlivými kauzálními faktory. Nebyl tak potvrzen předpoklad, že míra očekávání vlastní úspěšnosti souvisí se způsoby atribuování kauzality agrese. Respondenti obecně vykazovali vyšší míru self-efficacy. Výsledky dále poukázaly na to, že není rozdíl ve způsobu atribuování (z hlediska lokalizace) dle toho, kdo je považován za iniciátora agrese. Předpoklad, že klienti jako iniciátoři budou vykazovat více dispoziční faktory oproti zaměstnancům v roli agresora, nebyl potvrzen. Celkově pak byla zjištěna základní atribuční chyba při vysvětlování příčin vzniku agrese, a to bez ohledu na to, kdo je iniciátorem konfliktu. Respondenti tak přiřazovali příčinám spíše dispoziční, stabilní a kontrolovatelné faktory. V této souvislosti bylo důležitým zjištěním fakt, že respondenti označili v 70 % případech za agresora klienta. Tato fakta poukazují na tendenci zaměstnanců vnímat agresi na jejich pracovišti jako izolovaný jev, který vychází převážně z klientů a který z hlediska budoucnosti již nelze změnit (neboť jsou příčiny spíše dispoziční a stabilní). Agrese klientů je tak chápána jako součást jejich každodenní práce, kterou však dokáží zvládat (vysoká míra self-efficacy) díky svému profesionálnímu vyškolení v oblasti restriktivních opatření. K tomu, aby si zaměstnanci začali uvědomovat širší kontext vzniku agrese na pracovišti je pak třeba, aby změnili perspektivu, se kterou se na ni dívají. Zejména pak kladli důraz na životní podmínky, které mohly a mohou vést k maladaptivnímu jednání klienta, a na význam jejich interakce s klienty. Nenadsazování dispozičních příčin nad situačními pak představuje významný předpoklad k cestě ke změně.

Abstract:

In my work I focus on those potential sources which can initiate an inception of aggression in an institutional care of both parties; of people with intellectual disability and also of staff. I have accentuated the interaction context in which the aggressive behaviour in the institutional care takes place. I have carried out a research which deals with ways by which staff explain the aggression inception on their workplace. The research reclines on the attribution processes and influences which could determine the processes. I have paid special attention to self-efficacy of the staff. The research emanates from quantitative and qualitative approach; 55 concerned respondents reacted in written form to a provoking photography being asked to narrate a story based on the photography. Furthermore respondents were asked to fill in 2 scales. First scale concentrates on detection of attribution factors according to Heider typology (locus, stability and controllability), second scale aims at determination of the level of self-efficacy of the respondent in the area of client´s challenging behavior management. There was no connection found between the level of self-efficacy of the staff and individual causal factors according to the results. The premise that a level of expectation of one´s own success is connected to the means of attribution of causality of aggression was not confirmed. The respondents generally embodied a higher level of self-efficacy. Furthermore the results point at the fact that there is no difference in the way of attribution (from the localization point of view) between different initiators of aggression. The premise that clients as initiators will embody more dispositional factors in comparison to staff in the roles of aggressors was not affirmed. On the whole, the fundamental attribution error during explanation of the aggression inception regardless the initiator of the conflict was discovered. The respondents then perceived the cause of aggression being due to the internal, stable and controllable factors. In context of that an important discovery was made that in 70% of the respondents´ answers the client was identified as the aggressor. These facts advert at the tendency of the staff to perceive aggression on their workplace as an isolated phenomenon which arises prevailingly from clients and which is impossible to alter in the future (due to their conviction that the causes of aggression are rather of dispositional and fixed nature). Aggression of clients is understood as a part of everyday work, which the staff is capable of managing (high level of self-efficacy) thanks to their professional training in the restrictive measures area. To make the staff become aware of a broader context of the inception of aggression on the workplace, it is essential for them to change their perception of aggression, i.e. to emphasize the environmental conditions which can lead to maladaptive acting of a client and also to stress out the importance of their interaction with clients. Avoiding the exaggeration of the internal causes above the external ones then represents a significant presupposition of the way of alternation.
