Indications for videofluoroscopic examination of swallowing and evaluation of results in the diagnosis of swallowing disorders in children

Abstract:

U rizikových skupin dětí je nutno předpokládat možnost aspirace potravy do dýchacích cest, zvýšeně je v tomto ohledu sledovat a aktivně u nich zjišťovat možné klinické známky aspirace komplexním postupem: anamnézou, klinicko-logopedickým vyšetřením a lékařským vyšetřením polykání. Tato studie upozorňuje na fakt, že zejména u neurologických dětských pacientů jsou jejich aspirace většinou tiché a aspirační pneumonie může být prvním projevem dysfagie. U těchto pacientů je proto nezbytné aktivně prokazovat tichou aspiraci a zjišťovat její predisponující stavy jako jsou penetrace a tvorba hypofaryngeálních reziduí. Podle našeho vlastního pozorování se u dětí s neurologickou diagnózou aspirace nemusí manifestovat klinickými příznaky. Ideálem diagnostických snah je prokázání aspirací a/nebo jejich predisponujících nálezů ještě před epizodou aspirační pneumonie, a pokusit se jim terapeuticky zabránit. Zjišťování specifických klinických symptomů dysfagie je spolehlivou a účinnou metodou v managementu dysfagie u novorozenců a malých dětí a může nám pomoci identifikovat pacienty indikované k dalšímu vyšetření jako je videofluoroskopie , která je zlatým standardem k prokázání deglutivních aspirací. Dostatečně rychlý nástup diagnostiky a terapie může ochránit děti před život ohrožujícími komplikacemi.

Abstract:

The aim of the work is to point out the need to assume the possibility of aspiration in high-risk groups of children, to monitor them in this respect and to actively detect possible clinical signs of aspiration in a comprehensive procedure: anamnesis, clinical-speech therapy and medical swallowing. The study draws attention to the fact that especially in neurological pediatric patients, their aspirations are mostly silent and aspiration pneumonia may be the first manifestation of dysphagia. It is therefore necessary to actively demonstrate silent aspiration and to detect its predisposing states / penetration and residue formation / especially in neurological patients. According to our own observations, children with a neurological diagnosis of aspiration may not show clinical signs. The ideal diagnostic effort is to demonstrate aspirations (and / or their predisposing findings-penetration and residues) before an episode of aspiration pneumonia, and to try to prevent them therapeutically. Detection of specific clinical symptoms of dysphagia is a reliable and effective method in the management of dysphagia in neonates and young children and can help us select patients indicated for further examination such as VFSS. This method is then the gold standard for demonstrating deglutant aspirations. The main purpose of this communication is to highlight the need for rapid diagnosis and therapy, as it can protect children from preventable life-threatening complications.
