Organization of the Clinical hematology laboratory during the COVID-19 pandemic

Abstract:

Pandemie COVID-19 ovlivnila prakticky všechny úseky zdravotnického systému včetně laboratorní diagnostiky. Kromě základních infekčních markerů se začaly provádět hematologické metody pro monitoraci zdravotního stavu hospitalizovaných pacientů souvisejících s působením viru v organismu. Cílem diplomové práce je zhodnocení změn a situace pomocí SWOT analýzy, zavedení metody Anti-Xa pro monitoring vysokomolekulárního heparinu do provozu, a porovnání prováděných hematologických metod u nakažených pacientů s následným vypracováním finanční analýzy. Výsledkem porovnání je celkové snížení počtu vyšetření první rok pandemie (2020), naopak nárůst v roce 2021. Potvrdil se význam u metod: D-dimery, fibrinogen, Anti-Xa pro LMWH i UFH, a ISIC skóre. SWOT analýza zpětně odhalila příležitosti a hrozby vyplývající ze slabých a silných stránek laboratoře.

Abstract:

The COVID-19 pandemic has affected virtually all parts of the healthcare system, including laboratory diagnostics. In addition to basic infectious markers, haematological methods have begun to be performed to monitor the health status of hospitalised patients related to the virus in the body. The aim of this thesis is to evaluate the changes and the situation by means of SWOT analysis, the introduction of the Anti-Xa method for the monitoring of high molecular weight heparin into operation, and the comparison of the performed hematological methods in infected patients, followed by a financial analysis. The comparison resulted in an overall decrease in the number of tests in the first year of the pandemic (2020), and an increase in 2021. The importance of the methods was confirmed: D-dimers, fibrinogen, Anti-Xa for both LMWH and UFH, and ISIC score. SWOT analysis retrospectively revealed opportunities and threats arising from the laboratory's strengths and weaknesses.
