Evaluation of selected parameters potentially influencing conception in young patients undergoing embryo transfer

Abstract:

Rigorózní práce je zaměřena na léčbu neplodnosti metodami asistované reprodukce, konkrétně in vitro fertilizaci (IVF), při které dochází k přenosu embrya do těla ženy. Teoretická část se zabývá problematikou reprodukčního stárnutí, hormonální regulací rozmnožování, vzniku a příčinami neplodnosti, rizikovými faktory a shrnuje diagnostiku a současné možnosti léčby metodami asistované reprodukce včetně farmakoterapie. Výzkumnou část tvoří analýza vybraných parametrů dostupných ze zdravotní dokumentace pacientek, které by mohly mít potenciální vliv na úspěšnost otěhotnění. Cílovou skupinu představují mladé ženy do 35 let podstupující IVF cyklus za účelem embryotransferu. V praktické části je hodnoceno srovnání efektu na vznik a udržení těhotenství pomocí ukazatelu Pregnancy rate v závislosti na věku pacientek, počtu absolvovaných cyklů IVF, počtu přenesených embryí a době jejich kultivace, užití mikromanipulační metody a luteální podpoře. Dále je zkoumáno zastoupení a vliv diagnózy neplodnosti na úspěšnost otěhotnění, vzájemný vztah mezi parametry charakterizujícími ovariální funkci, četnost výskytu porodů, potratů, mimoděložních těhotenství a jiných onemocnění v anamnéze.

Abstract:

Rigorous work is focused on the treatment of infertility by methods of assisted reproduction, specifically in vitro fertilization (IVF), which involves the transfer of a fertilized embryo into a woman's body. The theoretical part deals with the issue of reproductive aging, hormonal regulation of reproduction, the origin and causes of infertility, risk factors, and summarizes the diagnosis and current treatment options including pharmacotherapy. The research part consists of an analysis of selected parameters extracted from the medical documentation of a set of patients, which could have a potential impact on the success of conception. The target group is young women under the age of 35 undergoing embryo transfer. The practical part evaluates the comparison of the effect on the inception and sustainment of pregnancy by observing how the indicator Pregnancy rate is influcenced by age of patients, number of completed IVF cycles, number of transferred embryos and time of their cultivation, use of micromanipulation method, luteal support. Furthermore, it is examined how distribution and causation of the infertility diagnosis, the relationship between the parameters characterizing ovarian function, and the quantity of births, abortions, ectopic pregnancies, and other history of diseases impact the likehood of successful conception.
