Contribution of Next-Generation Sequencing in the diagnosis of inherited cardiac diseases

Abstract:

Diplomová práce se věnuje problematice diagnostiky dědičných kardiologických onemocnění metodami masivně paralelního sekvenování (MPS). Tyto metody umožňují paralelní analýzu více vzorků a více oblastí genomů v jedné analýze, což vede ke zrychlení procesu diagnostiky dědičných srdečních onemocnění, která vzhledem k heterogenitě těchto onemocnění byla dříve omezená jen na pár vybraných genů. V současné době, kdy jsou molekulárně biologické techniky analýzy genomu využívány v klinické praxi, se otevírají nové možnosti diagnostiky komplexních chorob. Analýzou genomu byly vytipovány desítky genů, které se mohou podílet na rozvoji dědičných kardiologických onemocnění. Použití MPS však sebou přináší i určitá úskalí v podobě obrovského nárůstu dat, jejichž interpretace není vždy jednoznačná. Tato diplomová práce se soustřeďuje na problematiku interpretací získaných variant a jejich reklasifikaci podle neustále se měnících informací ve světových databázích. Dále se zaměřuje na posouzení kauzality klasifikovaných variant pomocí segregační analýzy v rodinách probandů.

Abstract:

This diploma thesis is focused on the diagnosis of hereditary cardiological diseases by methods of massive parallel sequencing (MPS). These methods allow the parallel analysis of multiple samples and multiple genomic areas in one analysis, which accelerates the process of hereditary heart disease diagnosis. That was in past, due to the heterogeneity of these conditions limited to an only a few selected genes. Nowadays, when molecular biology techniques of genome analysis are used in clinical practice, new diagnostics options of complex diseases take on their role. Analyzing the genome, dozens of genes involved in the development of inherited cardiac diseases have been selected. However, the use of MPS brings some pitfalls in the form of a huge increase of data with unclear interpretation. This diploma thesis focuses on the interpretation of the acquired variants and their reclassification according to constantly changing information in the world databases. It also focuses on assessment of variants causality using segregation analysis in proband families.
