Physiotherapy for patients after transverse spinal cord lesion

Abstract:

Bakalářská práce s názvem Fyzioterapie u pacientů po transverzální lézi míšní obsahuje teoretickou a hlavní praktickou část. V teoretické části je popsána anatomie páteře, míchy a nervového systému. Dále je zde charakteristika míšní léze a jejích komplikací, které se u pacientů po míšní lézi vyskytují také je zde stručný popis fyzioterapeutických metod a konceptů. Praktická část bakalářské práce se zaměřuje na problematiku nesprávných pohybových stereotypů a na odstranění bolestí vzniklých kvůli jizvě po splenektomii. Praktická část obsahuje kazuistiku pacienta po transverzální lézi míšní, vstupní a výstupní kineziologický rozbor na základě kterých je sestaven krátkodobý a dlouhodobý plán fyzioterapie. Cílem práce je přispět k řešení problematiky fyzioterapie po transverzální lézi míšní. V bakalářské práci je vypracován vstupní kineziologický rozbor, který je složen z vyšetření aspekcí, palpací, antropometrického a goniometrického měření a neurologického vyšetření. Ze získaných údajů je navržen krátkodobý fyzioterapeutický plán. Fyzioterapeutická intervence trvala 10 týdnů, terapie probíhala jednou týdně a délka jedné terapie byla 60-90 minut. V rámci poslední intervence byl proveden výstupní kineziologický rozbor na základě, kterého byly zhodnoceny výsledky celé terapie. Z výsledků výstupního kineziologického rozboru byl navržen dlouhodobý plán fyzioterapie. Zhodnocením výsledků bylo zjištěno zvětšení rozsahu pohybu v daných kloubech, uvolnění některých hypertonických svalů, snížení intenzity bolestivosti jizvy po splenektomii a zjištění pozitivních změn v rámci antrometrického vyšetření hrudníku. Pro pacienta je důležité udržení správných pohybových stereotypů, které je možné udržovat v rámci autoterapie.

Abstract:

The bachelor thesis entitled Physiotherapy for patients after transverse spinal cord injury contains a theoretical and main practical part. The theoretical part describes the anatomy of the spine, spinal cord and nervous system. Furthermore, there is a characteristic of the spinal cord lesion and its complications, which occur in patients after spinal cord injury, there is also a brief description of physiotherapeutic methods and concepts. The practical part of the bachelor thesis focuses on the issue of incorrect movement stereotypes and the elimination of pain caused by a sclenectomy scar. The practical part contains a case report of a patient after a transverse spinal cord injury, input and output kinesiological analysis on the basis of which a short-term and long-term physiotherapy plan is compiled. The aim of this work is to contribute to the solution of physiotherapy after a transverse spinal cord injury. The bachelor's thesis deals with an initial kinesiological analysis, which consists of examination of aspects, palpations, anthropometric and trigonometric measurements and neurological examination. A short-term physiotherapy plan is proposed from the obtained data. The physiotherapy intervention lasted 10 weeks, the therapy lasted once a week and the duration of one therapy was 60-90 minutes. As part of the last intervention, a final kinesiological analysis was performed, on the basis of which the results of the entire therapy were evaluated. A long-term physiotherapy plan was proposed from the results of the final kinesiological analysis. The evaluation of the results revealed an increase in the range of motion in the joints, relaxation of some hypertonic muscles, a reduction in the intensity of the pain of the scar after splenectomy and the detection of positive changes in the antrometric examination of the chest. It is important for the patient to maintain the correct movement stereotypes that can be maintained in autotherapy.
