Case study of women with BRCA 1 gene mutations in germ

Abstract:

Cíl: Cílem práce bylo vypracování komplexní kazuistiky ženy, u které sice byla zjištěna genová mutace BRCA1, ale nebylo u ní diagnostikováno nádorové onemocnění prsu a také dcery naší respondentky, která po prodělání nádorového onemocnění zjistila, že trpí stejnou mutací genu. Zároveň bylo provedeno srovnání obou žen v přístupu k prevenci, léčbě onemocnění a v samotném prožívání zjištěné diagnózy. Metodika: Empirická část je rozdělena na tři části. Anamnézu matky a dcery, která obsahuje stručný úvod a základní informace o respondentce a její dceři. Katamnézu, která je rozpracována do tří tematických okruhů, podle období v životě žen: první období je období před zjištěním diagnózy genové mutace a vyrovnávání se s onemocněním dcery, druhé období je období prožívání genetických testů a zjištění genové mutace, a třetí období je období po genetickém testování a vyrovnávání se s touto diagnózou. Výsledky: V rozhovoru s oběma ženami bylo zjištěno, že jejich přístup k problematice se zásadně liší. U starší ženy se dá říci, že už od mladého věku dodržuje zásady správné primární prevence, naproti tomu dcera nikoliv. Zároveň gynekologická anamnéza vychází v neprospěch mladší ženy. Také přístup k prevenci sekundární, převážně k samovyšetřování prsu, je u obou žen rozdílný. Je možné, že zmiňované faktory mohly přispět k tomu, že dceru nádorové onemocnění postihlo a matku nikoliv. Závěr: Dědičně podmíněná nádorová onemocnění prsu a vaječníků tvoří přibližně 5-10% ze všech diagnostikovaných nádorů. Na jejich vzniku se ve vice než polovině případů podílejí zárodečné mutace genů BRCA1 nebo BRCA2. Rodiny s hereditárním výskytem těchto nádorů vyžadují specifickou péči ve specializovaných centrech. Pokud je u ženy odhalena mutace těchto genů, je možné zaujmout různé postoje, z nichž jsou nejprospěšnější: zvýšená bdělost a provádění preventivních prohlídek, změna životního stylu s cílem omezit vliv dalších rizikových faktorů, posílit preventivní faktory, absolvovat preventivní operace.

Abstract:

Aim: The aim was to develop a comprehensive case histories woman who was discovered while the BRCA1 gene mutation, but was diagnosed with breast cancer and daughters of our respondents, which was found in the same gene affected, unfortunately, to cancer. At the same time, a comparison of the two women in access to prevention, treatment of disease and the very experience of the diagnosis. Methodology: The empirical part is divided into three parts. A history of mother and daughter, which includes a brief introduction and background information on the respondent and her daughter. Katamnézu, which are divided into three thematic areas in the lives of women: the first period is the period before diagnosis of the diagnosis of gene mutations and coping with illness daughter, the second season is the experience of genetic testing and identification of gene mutations, and the third period is the period after genetic testing and coping with this diagnosis. Results: In an interview with two women found that their approach to the issue is fundamentally different. For older women, we can say that from a young age, observe the principles of good primary prevention, whereas the daughter does not. Gynecological history also comes to the detriment of younger women. Also, access to secondary prevention, mainly for breast self-examination, both women are different. It is possible that these factors may contribute to the cancer affected daughter and mother are not. Conclusion: Hereditary breast cancer and ovarian cancer account for approximately 5-10% of all cancers diagnosed. At its inception, more than half the cases involved in germline mutations of BRCA1 or BRCA2. Families with hereditary occurrence of these tumors requires specific care in specialized centers. If a woman is discovered mutations of these genes, it is possible to take different positions from which they are most beneficial: increased vigilance and preventative checks, lifestyle changes to reduce the influence of other risk factors, to strengthen preventive factors, undergo preventive surgery.
