Optimální postupy při operačním řešení dekubitů - kvalita kožního krytu po rekonstrukčních operacích

Optimal practice in surgical treatment of pressure sores

Abstract:

Chirurgické řešení dekubitů za použití metod plastické chirurgie může být přínosem pro léčbu dekubitů III. a IV. stupně. Urychlením léčby se zmenší náklady na akutní péči a zlepší se kvalita života pacienta. Prevalence dekubitů III. a IV. stupně se v EU udává 8,7 %. U pacientů v dlouhodobé lékařské a ošetřovatelské péči roste až k 25 %. Při stanovení rizikových faktorů je velkým přínosem pak implementace preventivních opatřeních v rutinní ošetřovatelské praxi. Cílem této práce bylo stanovit indikace možných postupů pro operační řešení dekubitů na základě analýzy retrospektivního datového souboru 329 hospitalizací, experimentu na zvířeti a prospektivní studie. Při respektování obecných chirurgických pravidel a postupů plastické chirurgie jsme získali data souboru, kde můžeme konstatovat, že statisticky významně byly v převaze operovaní muži, kteří byli starší, ale průměrný věk byl 47,3 rok. Hospitalizace trvala průměrně 24,7 dne. V pelvické oblasti mělo defekt 76 % pacientů a při řešení převažovaly muskulokutánní a fasciokutánní laloky. Recidivy popisujeme u 26 % pacientů. Ve dvou etapách se operovalo u 65 % pacientů. Při propuštění bylo zhojeno 74 % operovaných. Výskyt recidiv je statisticky významně vyšší u mužů s delší první hospitalizací a transverzální míšní lézí. Zároveň výskyt recidiv statisticky nezávisel na věku, typu operačního řešení (použitého laloku) či doléčení pacienta na oddělení, ani jestli se jednalo o ischiální dekubit. Experimentálně bylo potvrzeno urychlené hojení defektu zvládnutím lokální infekce a za použití vlhké terapie. V prospektivní studii se nepotvrdil vliv jedno či dvouetapové léčby na frekvenci reoperací a recidiv. Námi prezentované výsledky mohou sloužit jako podklad pro pokračovaní sledovaní v této oblasti: rozšířit sledovaný soubor, pokračovat v zavedených indikacích a operačních postupech a rozšířit je o další terapeutické možnosti (laloky volné, tissue expandery, senzitivní laloky, stem cells). Dále by bylo vhodně ověřit na větším souboru platnost hypotézy, že jedno a dvoetapové operace dekubitů nemají vliv na vznik recidiv. Dále chceme participovat na formulaci a zavádění doporučených postupů při léčbě ran tak, aby progresivní léčebné postupy a holistický přístup k pacientovi byly implementovány v široké praxi.

Abstract:

The surgical solution of decubital ulcers employing plastic surgery methods can be a benefit to treatment of decubital ulcers of stages III and IV. Speeding up of the treatment reduces the cost of acute care and improves the quality of the patient’s life. The prevalence of stage III and IV decubital ulcers within EU is reported to reach 8.7 %. In patients under long-term medical and nursing care it increases to up to 25 %. When determining the risk factors a great benefit is created by implementing preventive measures to the routine nursing practice. The purpose of this study was to define indications for possible procedures of operations on decubital ulcers based on the analysis of a retrospective data file of 329 hospitalizations, an animal experiment and prospective study. While respecting the general surgical rules and plastic surgery procedures we obtained the data of the file where we can state that, in a statistically significant measure, the males operated on prevailed, generally of an older age, however their average age was 47.3 years. Hospitalization lasted 24.7 days on average. 76 % of patients had a defect in the pelvic area and musculocutaneous and fasciocutaneous lobes were prevalent during the solution. We have described relapses in 26 % patients, 65 % of patients were operated on in two stages. 74 % of patients operated on were healed completely on discharge from hospital. The occurrence of relapses is statistically more significant in males with a longer time of first hospitalization and transversal spinal lesion. At the same time, the relapse occurrence did not depend on age, type of operating solution (lobe used) or finishing treatment of patients in the ward, either on whether ischial decubital ulcer was involved. It was experimentally proved that the defect healed faster when the local infection was brought under control and wet therapy used. The prospective study has not confirmed effect of one- or two-stage treatment on the frequency of reoperations and relapses. The results presented by us can be used as a basis for continued monitoring of this area: extend the set monitored, continue to use established indications and operating procedures and extend them by further therapeutic possibilities (free lobes, tissue expanders, sensitive lobes, stem cells). Further it would be useful to verify, on a larger set of patients, validity of the hypothesis saying that one- and two-stage operations of decubital ulcers do not affect relapse occurrences. We also wish to participate in formulating and introducing recommended procedures for wound treatment so that progressive treatment procedures and holistic approach to patients are implemented in general practice.
