Physiotherapy after the plastice LCA

Abstract:

Název: Fyzioterapie po plastice LCA. Cíl: Cílem práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u vybraného klienta po rekonstrukčním zákroku v oblasti kolenního kloubu. Metodika: Zpracovala jsem kazuistiku 20letého pacienta s diagnózou stav po plastice LCA. Je zaznamenána fyzioterapeutická intervence v pooperačním období v délce čtyř a půl měsíce. Je zpracován vstupní kineziologický rozbor a na jeho podkladě stanoven krátkodobý plán. Popisuji ošetření v průběhu šesti týdenní ambulantní fyzioterapie, kdy jsem sledovala parametry rozsah pohybu, svalová síla a stabilita kolenního kloubu. V terapii jsem v začátku použila prostředky jako polohování, kryoterapie, cévní gymnastika, techniky měkkých tkání, Míčková facilitace dle Zdeny Jebavé, cvičení v uzavřeném kinematickém řetězci a PFS. Dále jsem pokračovala v aktivním cvičení v otevřených kinematických řetězcích, PNF, cvičení s overballem a therabandem a dalšími pomůckami pro senzomotorické cvičení. V rámci poslední ambulantní terapeutické intervence jsem provedla výstupní vyšetření, na jehož podkladě byl stanoven dlouhodobý fyzioterapeutický plán a doporučena režimová opatření, spolu s doporučením optimální sportovní zátěže. Ve své práci jsem dále monitorovala funkční stav kolenního kloubu ještě tři měsíce. Bylo provedeno kontrolní vyšetření zaměřené na rozsah pohybu a svalovou sílu. Došlo k porovnání parametrů v tomto časovém odstupu a s pacientem byla konzultována sportovní zátěž. Výsledky: Na základě fyzioterapeutické intervence v rámci krátkodobého plánu došlo k obnově rozsahu pohybu, zvýšení svalové síly a úpravě stereotypu chůze. V rámci dlouhodobého fyzioterapeutického plánu a režimovým opatřením došlo k stabilizaci kolene, zlepšení celkového funkčního stavu a návratu ke sportovním aktivitám. Závěr: Práce mě obohatila o poznatek, že díky včasné a správně vedené fyzioterapii spolu s dobře spolupracujícím pacientem, je možno dosáhnout optimálních výsledků při fyzioterapii po plastice LCA i přes malé počáteční komplikace, které u pacienta nastaly. Pro mého pacienta měla terapie pozitivní efekt i díky jeho motivaci a dobré spolupráci byl možný bezproblémový návrat ke sportu, což je pro něj velmi důležité i z hlediska psychosociálního.

Abstract:

Title: Physiotherapy after ACL reconstruction. Aim: The goal of this bachelor thesis is to contribute to solving the problems of selecting optimal therapeutic methods, concepts and methods for a selected client after reconstructive surgery on the knee joint. Methodology: I processed a case report of a 20- year- old patient diagnosed with ACL plastic status. Physiotherapeutic intervention is recorded in the postoperative period of four and half mounths. An initial kinesiological analysis is prepared and based on a short-term plan. I describe the treatment during the six-week ambulatory physiotherapy when I tracked the parameters of the range of motion, muscle strength and stability of the knee joint. In the beginning, I used therapies such as positioning, cryotherapy, vascular gymnastics, soft tissue techniques, Zdena Jebavá's Ball Facility, closed kinematic exercise, and PFS. I continued with active exercise in open kinematic chains, PNF, exercise with overball and theraband and other aids for sensorimotor exercise. Within the last ambulatory therapeutic intervention, I performed an outpatient examination based on a long-term physiotherapy plan and recommended regimes, together with recommendations for optimal exercise load. In my work, I further evaluated the functional status of the knee joint yet tree months, when a control examination focused on the range of motion and muscle strength performed. Parameters of this time interval were compared, and the patient was consulted on his sports load. Within the long-term physiotherapy plan and regimen, the knee was stabilized, the overall functional status improved, and the patient has been able to return to sports activities. Conclusion: The work has enriched me with the knowledge that thanks to timely and wellmanaged physiotherapy as well as a well-collaborating patient, it is possible to achieve optimal results after ACL reconstruction despite the minor initial complications that occurred to the patient. For my patient, the therapy has had a positive effect, and thanks to his motivation and good cooperation, a smooth return to sport was possible, which is also very important from a psychosocial point of view. Results: Based on the physiotherapeutic intervention within the short-term plan, the range of movement was increased, as well as muscular strength and the stereotyped gait adjustment.
