Abstract:

Práce se zabývá teorií a praxí psychosociální rehabilitace s lidmi s duševním onemocněním v poskytování služeb Týmu bydlení v občanském sdružení Fokus Praha. Jedním z mých cílů je představení teorie psychosociální rehabilitace a popis její praxe v poskytování služeb Týmu bydlení. Teorii dokládám na konkrétní praxi s uživateli služeb. Cílem je ukázat podobu praxe psychosociální rehabilitace na spolupráci se třemi uživateli služeb formou kazuistik a doloženými nástroji psychosociální rehabilitace (konkrétně vypracovaným rehabilitačním plánem, protikrizovým plánem a hodnocením spolupráce). Dalším cílem práce je ověření naplňování postupů rehabilitace z pohledu klientů. Chci si ověřit, zda spolupráce probíhá dle nastavených kritérií a jestli výsledek takto nastavené spolupráce vnímají klienti ve svém životě jako užitečný pro změnu, které chtějí ve službě dosáhnout.

Abstract:

The work deals with the theory and practice of psychosocial rehabilitation of people with mental illness in the provision of housing services team in the civic association Fokus Praha. One of my goals is to introduce the theory of psychosocial rehabilitation and a description of its practice in the provision of housing services team. I argue theory for a particular practice with my clients. The goal is to show the shape of practice psychosocial rehabilitation in partnership with clients through three case studies and proven tools psychosocial rehabilitation (specifically prepared rehabilitation plans, and evaluation of anti-crisis plan of cooperation). Another goal of work is to verify the implementation of rehabilitation procedures from the perspective of clients. I would like to verify that cooperation takes place according to set criteria and if the result so long as the collaboration clients perceive in their lives as useful for a change in the service that they want to achieve.
