Physiotherapy of pacient with intervertebral disc herniation

Abstract:

Abstrakt: Cíl: Cílem této práce je přispět k řešení problematiky fyzioterapie u pacientů s výhřezem meziobratlových plotének pomocí vhodných fyzioterapeutických postupů, konceptů a metod. Metodika: V hlavní části je zpracována kazuistika muže ve věku 48 let s výhřezem meziobratlové ploténky v bederní oblasti. Na základě vstupního kineziologického vyšetření, kde jsem u pacienta využila aspekci, antropometrické měření, vyšetření hybnosti, zkrácených struktur, svalový test, neurologické vyšetření a testy na HSS jsem stanovila krátkodobý fyzioterapeutický plán. Poté následovala terapie, která probíhala 6 týdnů. Každý týden byla provedena fyzioterapeutická intervence pacienta, které trvala 60 minut. V terapii jsem nejvíce využívala techniku měkkých tkání, PFS, PIR, metodu ACT? a DNS. Po 6 týdnech byl zpracován výstupní kineziologický rozbor a na jeho základě byl stanoven dlouhodobý fyzioterapeutický plán. Výsledky: Již během terapie došlo u pacienta ke snížení bolestivosti. Dále po zpracování výstupního kineziologického vyšetření došlo ke zlepšení rozvíjení páteře. Schoberova, Stiborova vzdálenost se zlepšila o 0,5cm, Thomayerova vzdálenost o 3 cm, Ottova inklinační i reklinační vzdálenost o 1 cm a pohyb do lateroflexe se zlepšil na každou stranu též o 1 cm. Dále došlo k zvětšení rozsahu pohybu kyčelního kloubu do extenze z 5° na 10°. U pacienta také došlo ke zlepšení svalové síly, kde nastala změna hlavně v extenzi trupu, kdy před terapii ji pacient nebyl schopen provést pro bolest, po terapii jsem svalovou sílu hodnotila 2 stupněm. U zkrácených svalů došlo u flexorů kyčelního kloubu, flexorů kolenního kloubu, m. pectoralis major a paravertebrálních svalů ke zlepšení o 1 stupeň. Dále se také pacientovi zlepšila citlivost v dermatomu L4 a L5. Dle mého názoru tedy u pacienta došlo k nápravě patologických změn, které byli zjištěny vstupním kineziologickým vyšetřením. Závěr: Mnou zvolená fyzioterapeutická léčba u pacienta byla úspěšná. Díky této bakalářské práci jsem si sama rozšířila obzory přístupu fyzioterapeutické intervence u této problematiky.

Abstract:

Title of the research: Physiotherapy of pacient with intervertebral disc herniation Goal: This work aims to contribute to the solution of physiotherapy in patients with intervertebral disc herniation using appropriate physiotherapeutic procedures, concepts, and methods. Method: The main part deals with a case report of a man aged 48 years with prolapse of the intervertebral disc in the lumbar region. Based on the initial kinesiological examination, where I used the patient's aspect, anthropometric measurement, examination of momentum, shortened structures, muscle test, neurological examination, and tests for HSS, I determined a short-term physiotherapy plan. This was followed by therapy for 6 weeks. A patient check-up was performed physiotherapeutic intervention every week for 60 minutes. In therapy, I mostly used the technique of soft tissues, PFS, PIR, ACT? method, and DNS. After 6 weeks, a final kinesiological analysis was performed and a long-term physiotherapy plan was determined based on it. Results: Already during therapy, the patient had a reduction in pain. Furthermore, after the processing of the output kinesiological examination, the development of the spine was improved. Schober's, Stibor's distance improved by 0.5 cm, Thomayer's distance by 3 cm, Otto's inclination and reclining distance by 1 cm and movement into the lateroflex also improved on each side by 1 cm. Furthermore, the range of motion of the hip joint increased to an extension from 5 ° to 10 °. The patient also had an improvement in muscle strength, where there was a change mainly in the extension of the torso, when before the patient was unable to perform it for pain, after the therapy I evaluated the muscle strength by 2 degrees. In shortened muscles, the flexors of the hip joint, flexors of the knee joint, pectoralis major, and paravertebral muscles improved by 1 degree. In addition, the patient's improvement in L4 and L5 dermatome improved. In my opinion, the patient's pathological changes were corrected, which were detected by the initial kinesiological examination. Conclusion: The physiotherapeutic treatment of my choice in the patient was successful. Thanks to this bachelor's thesis, I broadened the horizons of the approach of physiotherapeutic intervention in this area.
