Use of modern technologies in management of children´s preoperative anxiety

Abstract:

Cílem přehledové bakalářské práce bylo vyhledat a analyzovat výsledky výzkumů, vědecké studie a dostupné informace týkající se problematiky využití moderních technologií v managementu předoperační úzkosti u dětí. Studie byly vyhledány systematickou rešeršní strategií pomocí předdefinovaných klíčových slov, zahrnovacích a vylučovacích kritérií za užití elektronických databází PubMed, MEDLINE Complete, CINAHL Plus with Full Text, Science Direct, ProQuest, Directory of Open Access Journals, OpenAIRE a BiblioMedica/Medvik. Extrakce, analýza a syntéza dat proběhla na základě stanovených dílčích cílů práce. Bakalářská práce identifikovala několik možností využití moderních technologií v edukačních i behaviorálních ošetřovatelských intervencích cílících na mírnění předoperační úzkosti u dětí. Bylo zjištěno, že jak edukační videa přizpůsobená dětským pacientům, tak virtuální prohlídky operačního sálu vykazují slibnou účinnost, zatímco efektivita přípravných předoperačních multimediálních aplikací a komplexních internetových přípravných programů zůstává sporná. Diskutováno bylo také využití videí za účelem odvedení pozornosti dítěte, což se jeví přinejmenším stejně efektivně jako anxiolytická premedikace midazolamem. Za nejúčinnější ošetřovatelskou intervenci k redukci předoperační úzkosti u dětí lze ovšem jednoznačně označit hraní počítačových a interaktivních her, a to jak s edukačním obsahem v rámci předoperační přípravy, tak i zábavných dle vlastního výběru dítěte k odvedení pozornosti dítěte před operací a při úvodu do anestezie. Efektivita této intervence je patrně dána schopností her vtáhnout dítě do děje, a tím odpoutat jeho pozornost od vnější reality, byť vztah mezi mírou ponoření se do aktivity a redukcí úzkosti je teprve předmětem výzkumu. Výsledky přehledové práce lze využít např. při rozhodování o případném pořízení moderních technologií k mírnění předoperační úzkosti dětí ve zdravotnických zařízeních.

Abstract:

The aim of the bachelor thesis was to find and analyse the results of research, scientific studies, and available information about the use of modern technologies in the management of children´s preoperative anxiety. Studies were searched by a systematic search strategy using predefined keywords, inclusion and exclusion criteria using the electronic databases PubMed, MEDLINE Complete, CINAHL Plus with Full Text, Science Direct, ProQuest, Directory of Open Access Journals, OpenAIRE and BiblioMedica/Medvik. The data were extracted, analysed, and synthetised in accordance with the sub-aims of the thesis. The bachelor thesis identified several possibilities of using modern technologies in educational and behavioural nursing interventions aimed at alleviating preoperative anxiety in children. Both educational videos tailored to paediatric patients and virtual tours of the operating theatre have been found to show promising efficacy, while the effectiveness of preparatory preoperative multimedia applications and comprehensive Internet preparation programs remains questionable. The use of videos to distract the child has also been discussed, which appears to be at least as effective as the anxiolytic premedication with midazolam. However, the most effective nursing intervention to reduce preoperative anxiety in children can clearly be identified as playing computer and interactive games, both with educational content as part of the preoperative preparation, as well as entertaining ones of the child's own choice to divert the child's attention before the operation and during the introduction to anaesthesia. The effectiveness of this intervention is probably due to the ability of the games to draw the child into the story and thereby distract his attention from external reality, although the relationship between the degree of immersion in the activity and the reduction of anxiety is still being researched. The results of the bachelor thesis can be used, for example, when deciding on possible acquisition of modern technologies to alleviate pre-operative anxiety of children in hospitals.
