Physiotherapy for painful menstruation

Abstract:

Úvod: Tématem této bakalářské práce je ovlivnění bolestivé menstruace za pomoci fyzioterapeutických metod. Cíl práce: Přispět k řešení problematiky fyzioterapie u pacientky s bolestivou menstruací, a to pomocí vhodně zvolených fyzioterapeutických metod, postupů a konceptů. Metodika: Hlavní část bakalářské práce popisuje kazuistiku 25leté pacientky s diagnózou N 94. - Bolest a jiné stavy sdružené s ženskými pohlavními orgány a menstruačním cyklem. Před zahájením fyzioterapeutické intervence byla odebrána anamnéza, vyplněn Menstrual Symptom Questionnaire (MSQ), proveden vstupní kineziologický rozbor a vstupní vyšetření pánevního dna na základě kterých byl sestaven krátký fyzioterapeutický plán probíhající po dobu 8 týdnů s frekvencí intervencí dvakrát týdně po dobu jedné hodiny v ambulanci fyzioterapie. V rámci jednotlivých intervencí bylo využito prvků respirační fyzioterapie, techniky měkkých tkání (TMT), mobilizace dle Lewita, postizometrické relaxace (PIR), dynamické neuromuskulární stabilizace (DNS), metody Ludmily Mojžíšové, akrální koaktivační terapie (ACT), Kegelových cviků. Při deváté intervenci bylo provedeno kontrolní vyšetření hlavních patologií. Po ukončení fyzioterapeutické intervence byl proveden výstupní MSQ spolu s výstupním kineziologický rozborem a výstupní vyšetření pánevního dna, na jejichž základě jsem navrhla dlouhodobý fyzioterapeutický plán. Výsledky: Během spolupráce s pacientkou jsem docílily výrazného zmírnění intenzity bolestí během jejího menstruačního cyklu, a to z původních 7 bodů na 4 body na vizuální analogové škále (VAS). Dále jsme pozitivně ovlivnily její dechový stereotyp, rozvíjení páteře, posílily jsme HSSP a pánevní dno. Terapie se kladně projevila i z hlediska celkového fyzického i psychického stavu pacienta. Závěry: Vlivem mé intervence se sice nepodařilo úplně odstranit bolesti spojené s menstruačním cyklem, ale podařilo se pozitivně ovlivnit intenzitu bolesti před a během menstruace. Vzhledem ke zlepšení stavu pacientky i jiných ohledů jsem přesvědčena o tom, že fyzioterapeutická intervence byla úspěšná.

Abstract:

Introduction: The topic of this thesis is the therapy of painful menstruation using physiotherapeutic methods. The objective: The aim of this thesis is to contribute to addressing the issue of physiotherapy in a patient with painful periods, using appropriately chosen physiotherapy methods, procedures and concepts. Methodology: Before starting the physiotherapy intervention, I performed the initial kinesiological examination and the initial pelvic floor examination, on the basis of which I developed a short physiotherapy plan lasting for 8 weeks with a frequency of inventions twice weekfor one hour. The Menstrual Symptom Questionnaire (MSQ) was also used before starting and ending the physiotherapy treatment. Using selected concepts and manual techniques, eliminate pathologies found in the input kinesiological analysis and strengthen the muscles of the deep stabilization system with a focus on the pelvic floor muscles. As part of the ninth intervention, perform a control examination of major pathologies. After completing the physiotherapy intervention, I performed an outgoing kinesiological analysis, on the basis of which I proposed a long-term physiotherapy plan. Results: During my cooperation with the patient, I achieved a significant reduction in the intensity of pain during her menstrual cycle, from the original 7 points to 4 points on the visual analog scale (VAS). Furthermore, we positively influenced her breathing stereotype, the development of the spine, we strengthened the HSSP and the pelvic floor. The therapy also had a positive effect in terms of the patient's overall physical and psychological condition. Conclusions: As a result of my intervention, it was not possible to completely eliminate the pain associated with the menstrual cycle, but it was possible to positively influence the intensity of the pain before and during menstruation. Given the improvement in the patient's condition in other respects, I am convinced that the physiotherapy intervention was successful.
