Systemic therapy of the client with depressive symptoms: Systematic case study

Abstract:

Práce je zaměřena na účinnost systemické psychoterapeutické léčby klienta (32 let), přicházejícího do terapie s depresivní symptomatikou. Jedná se o systematickou případovou studii. Sběr dat byl proveden během 20 sezení a byl součástí projektu, který proběhl pod záštitou Centra pro výzkum psychoterapie (CVP). Cílem práce je zodpovědět, zda došlo u klienta ke změně a pokud ano, zda je možné považovat psychoterapii za původce těchto změn. Posledním bodem je poté uvedení faktorů, které byly v rámci psychoterapie nápomocné pro klientovu změnu. Pro výzkumnou část byla využita metoda Hermeneutic Single Case Efficacy Design (HSCED). Metoda zahrnuje hodnocení nasbíraných kvalitativních a kvantitativních dat z pozice zastávající účinnost terapie a na druhou stranu zároveň z pohledu někoho, kdo je k takovým výsledkům skeptický. Následným krokem je konečné posouzení terapie. Tato metoda je označována jako validní zdroj důkazu o efektivitě konkrétní terapie. Z výsledků je patrné, že v případě klienta v této systematické případové studii došlo k terapeutické změně, a to na základě právě posouzení kvalitativních a současně kvantitativních ukazatelů. Zároveň je možné potvrdit, že klientova změna vychází z jeho práce v psychoterapii. Následně jsou uvedeny faktory, které v terapeutickém procesu měly na klientovu změnu vliv. Mezi ně patří dobrá terapeutická aliance spolu s terapeutovým sebeodhalením, nedirektivním přístupem a terapeutovou podporou. Zároveň byly jako nápomocné vyhodnoceny konkrétní psychoterapeutické intervence.

Abstract:

The work is focused on the effectiveness of systemic psychotherapeutic treatment of a client (32 years old) coming to therapy with depressive symptoms. This is a systematic case study. Data collection was performed during 20 sessions and was part of the project, which was carried out under the auspices of the Centre for Psychotherapy Research. The aim of this work is to answer whether the client has changed and if so whether psychotherapy can be considered as the cause of these changes. The last point is the stating of factors that were helpful in psychotherapy for the client's change. Hermeneutic Single Case Efficacy Design (HSCED) was used for the research. The method involves evaluation of the collected qualitative and quantitative data from a position that advocates the effectiveness of the therapy and, on the other hand, from the perspective of someone who is sceptical of such results. The next step is the final assessment of the therapy. This method is referred to as a valid source of evidence for the effectiveness of a particular therapy. The results show that in the case of the client in this systematic case study there was a therapeutic change based on the assessment of both qualitative and quantitative indicators. It is also possible to confirm that the client's change is based on his work in psychotherapy. Then, the factors that influenced the client's change in the therapeutic process are listed. These include a good therapeutic alliance along with the therapist's self-disclosure, non-directive approach and therapist support. Specific psychotherapeutic interventions were evaluated as helpful, as well.
