Therapeuts Experience of the Psychotherapeutic Alliance in treatment of patients with BPD

Abstract:

Práce se věnuje zkušenostem psychoterapeutického vztahu u psychoterapeutů v individuální psychoterapii s pacienty s diagnózou hraniční poruchy osobnosti. Bylo vybráno šest psychoterapeutů z klinické praxe, kteří s pacienty s touto diagnózou pracují, a s nimi bylo provedeny polostrukturované rozhovory doplněné o kresebnou techniku „čáry terapie“ jako modifikaci Čermákovy techniky „čáry léčby“. Výzkumu předcházela teoretická část ve formě přehledové studie, která mapuje základní přístupy ke konceptům psychoterapeutického vztahu a hraniční poruchy osobnosti. Nechyběl rovněž přehled obecných i speciálních psychoterapeutických přístupů k léčbě této poruchy. Z fenomenologické analýzy získaných výpovědí vyplývá vedle dalších tématat zejména tématika hranic mezi terapeutem a pacientem a vztahu rodič-dítě, kde terapie je symbolickým dospíváním pacienta. Výsledky podporují koncepty projektivní identifikace, štěpení nebo mentalizace (Kernberg, Fonagy ad.).

Abstract:

The final paper is based on evaluation of experience of six therapists working with patients diagnosed with Borderline Personality Disorder (BPD) in individual psychoterapy. The data were obtained through semi-structured interview accompanied by a modification of „life-line-method“ (Čermák). The theoretical part is a summary of different approaches towards a therapeutic alliance and diagnostic concept of BPD from different perspectives, describing each concept of therapeutic approach to patients with BPD. As a result of phenomenological analysis we got mostly topics of boundaries in the therapeutic alliance and a parent-child relationship with the progress in therapy as a symbolic growing up of the patient. The topics is in correspondancy with psychoanalytic theoretical concepts of projective identification, splitting and mentalization (Kernberg, Fonagy etc.).
