Abstract:

Úvod: Tématem mé bakalářské práce je terapie idiopatických skolióz pomocí Inovovaného Klappova lezení. Zaměřila jsem se na problematiku idiopatické skoliózy v dospělém věku, kdy pacientka s křivkou tvaru písmene C s úhlem 25 stupňů nebyla léčena korzetoterapií kvůli pozdnímu nálezu. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie dospělých pacientů s idiopatickou skoliózou, a to volbou vhodných fyzioterapeutických metod, postupů a konceptů. Metodika: V hlavní části mé práce jsem vyhotovila kazuistiku pětatřicetileté pacientky s neléčenou skoliózou. Před zahájením samotné terapie jsem zpracovala vstupní kineziologický rozbor, který obsahuje vyšetření aspekcí a palpací, antropometrii, dynamiku páteře, vyšetření svalového tonu, zkrácených svalů, svalové síly, hypermobility, kloubních blokád a dechového stereotypu. S pacientkou jsem poté měla deset terapií, na kterých jsme se věnovaly především Inovovanému Klappovu lezení. Při prvních dvou terapiích jsem použila mobilizaci krční páteře do rotace na pravou stranu, abych uvolnila blokádu v segmentu C2/3, mobilizaci hrudní páteře do extenze a lateroflexe, abych uvolnila blokádu v segmentu Th6/7. Před každým lezením jsem použila techniky měkkých tkání a postizometrickou relaxaci na uvolnění hypertonu paravertebrálních svalů v dolní oblasti hrudní páteře, proprioreceptivní neuromuskulární facilitaci pro posílení ochablých svalů a centraci ramenních kloubů dle Jarmily Čápové. Po pěti terapiích jsem vyhotovila kontrolní kineziologický rozbor a po absolvování všech deseti návštěv jsem vypracovala výstupní kineziologický rozbor a výsledky jsem použila k naplánování dlouhodobého rehabilitačního plánu. Výsledky: Postura mé pacientky se výrazně zlepšila, její asymetrie se částečně vyrovnaly a orientační vyšetření prokázalo i mírné srovnání křivky skoliózy. Došlo i k ústupu tupé bolesti zad. Výrazně se zlepšila svalová síla trupu a podařilo se částečně snížit hypertonus některých svalů. Závěry: Využitím výše uvedených technik jsem dosáhla zlepšení stavu pacientky. Pro udržení tohoto stavu je nutná každodenní fyzická aktivita. A pro zlepšení tohoto stavu je potřebné pokračovat ve fyzioterapeutické intervenci.

Abstract:

Introduction: The topic of my bachelor thesis is the therapy of idiopathic scoliosis using the Innovated Klapp method. I focused on the issue of idiopathic scoliosis in adulthood, when a patient with a C-shaped curve with an angle of 25 degrees was not treated with corset therapy due to a late diagnosis. The objective: The goal of the thesis is to contribute to the solution of the issue of physiotherapy for adult patients with idiopathic scoliosis by choosing appropriate physiotherapy methods, procedures and concepts. Methodology: In the main part of my thesis I created a casuistry of 35 years old patient with untreated scoliosis. Before starting the therapy itself, I made an initial kinesiological analysis, which includes inspection, palpation, anthropometry, spinal dynamics, examination of the muscle tone, shortened muscles, muscle strenght, hypermobility, joint blockages and respiratory stereotype. Then I had ten meetings with my patient, at which we mainly focused on the Innovated Klapp method. In the first two therapies I used cervical spine mobilisation into rotation to the right side to release the blockage in the C2/3 segment, mobilisation of the thoracic spine to extension and lateroflexion to release the blockages in the Th6/7 segment. Before each usage of the Innovated Klapp method, I used soft tissue techniques and postisometric relaxation to release the hypertension of paravertebral muscles in the lower area of thoracic spine, proprioreceptive neuromuscular facilitation to strenghten weakened muscles and centration of the shoulder joints according to Jarmila Čápová. After five therapies, I performed a control kinesiological analysis and after completing all ten therapies, I prepared a final kinesiological analysis and used the results to create a long-term rehabilitation plan. Results: My patient's posture has significantly improved, her asymmetries has partially evened out, and the indicative examination also showed a slight unbending of the scoliotic curve. The dull back pain also subsided. The muscle strenght of the trunk has improved significantly and the hypertonus of some muscles was partially reduced. Conclusions: Using the techniques I named above, I was able to improve the patient's condition. Daily physical activity is necessary to maintain this condition. And to improve this condition, it is necessary to continue the physiotherapeutical intervention.
