Involvement of non-coding RNAs in pathogenesis of renal cell carcinoma

Abstract:

Renální karcinom (renal cell carcinoma, RCC) se řadí mezi nejčastější malignity dospělého věku. Navzdory dramatickému pokroku v diagnostických a léčebných možnostech však stále není možné diagnostikovat pacienty s tímto onemocněním včas a velká část případů je zachycena až v metastatickém stádiu onemocnění. U pacientů v časných stádiích často dochází k relapsu i po úspěš-ném odstranění primárního tumoru. Současné možnosti jsou v předpovědi prognózy a léčebné odpovědi těchto pacientů výrazně limitované a nepostačují zejména pro výběr optimální léčebné strategie ze široké palety léků, které jsou dnes dostupné pro pacienty s metastatickým onemocněním. Nekódující RNA, významné endogenní regulátory genové exprese, jsou v posledních dvou deseti-letích intenzivně studovány nejen jako možné faktory zapojené do rozvoje mnoha onemocnění člověka včetně RCC, ale také jako potenciální biomarkery využitelné v klinické praxi. Zdaleka nejprostudovanějšími jsou mikroRNA (miRNA) a dlouhé nekódující RNA (long non-coding RNA, lncRNA), u kterých je již dnes známo mnoho kandidátních diagnostických, prognostických a prediktivních biomarkerů. Předkládaná disertační práce je komentovaným souborem publikací autorky, které vedly k rozšíření poznatků v této oblasti. Byl ověřen prediktivní potenciál několika miRNA, které byly již dříve popsány, z nichž nejlepší výsledky dosáhly miR-942 a miR-133. Dále byla na základě výsledků profilování miR-NA identifikována miR-376-3p se schopností odlišit pacienty s horší léčebnou odpovědí na sunitinib. Pomocí profilování lncRNA bylo zjištěno, že lncRNA PVT1 dokáže odlišit renální parenchym a renální nádorovou tkáň. Tato lncRNA byla dále prostudována pomocí in vitro funkčních testů, které poukázaly na zapojení PVT1 do rozvoje RCC. Výsledky prokázaly onkogenní vliv PVT1, což koresponduje s výsledky jiných autorů. Prokázali jsme, že PVT1 je slibný diagnostický biomarker a potenciálně také nový terapeutický cíl u pacientů s RCC.

Abstract:

Renal cell carcinoma (RCC) is one of the most common malignancies in adults. However, despite dramatic advances in diagnostic and treatment options, it is still not possible to diagnose patients with this disease in time, and a large proportion of cases are detected at the metastatic stage of the disease. Patients in earlier stages often relapse even after successful removal of the primary tumor. The current possibilities in the assessment of the prognosis and treatment response of these patients are significantly limited and are not sufficient, especially for the selection of the optimal treatment strategy from the wide range of drugs that are available today for patients with metastatic disease. Non-coding RNAs, important endogenous regulators of gene expression, have been exten-sively studied in the last two decades not only as possible factors involved in the development of many human diseases, including RCC, but also as potential biomarkers useful in clinical practice. By far the most studied are microRNAs (miRNAs) and long non-coding RNAs (lncRNAs), from which many potential diagnostic, prognostic and predictive biomarkers are already known. The submitted dissertation is a commented set of the author's publications, which led to the expansion of knowledge in this area. The predictive potential of several miRNAs previously described was validated, of which miR-942 and miR-133 achieved the best results. Furthermore, based on the results of miRNA profiling, miR-376-3p was identified with the ability to differentiate patients with a poorer response to sunitinib. Using lncRNA profiling, PVT1 lncRNA has been found to distinguish between renal parenchyma and renal tumor tissue. This lncRNA was further studied using in vitro functional assays that indicated the involvement of PVT1 in RCC development. The results showed an oncogenic effect of PVT1, which corresponds to the results of other authors. We have shown that PVT1 is a promising diagnostic biomarker and potentially a new therapeutic target in patients with RCC.
