Evaluation of influence of allergen-specific immunotherapy on consumption of symtomatic anti-allergic treatment

Abstract:

S pylovou alergií se potýká přibližně 25 % evropské populace, přičemž u některých pacientů, kteří mají až přehnaně silnou reakci, která není regulovatelná běžným používáním antihistaminik, se přistupuje ke specifické alergenové imunoterapii (SAIT), která pomáhá imunitnímu systému vybudovat si toleranci vůči danému alergenu. V této bakalářské práci se věnuji vlivu SAIT na další spotřebu farmakoterapeutik pomocí obvyklých denních terapeutických dávek (ODTD) pro pacienty s pylovou alergií. Mezi cíle práce patří rešerše informací o alergii a její léčbě, identifikace relevantních záznamů pro zadanou otázku a vlastní analýza datového souboru. Data pro zkoumání vlivu imunoterapie pochází z Národního registru hrazených zdravotních služeb a pro jejich analýzu byly použity statistické přístupy jako například párový a nepárový t-test, Wilcoxonův párový test a lineární regresní model. Výsledky analýzy ukázaly, že SAIT má vliv na další spotřebu antihistaminik, a to ve smyslu zmenšení potřebných dávek ODTD po skončení léčby. Zajímavé je, že ke snížení spotřeby antihistaminik dochází neustále i po skončení SAIT. Dále výsledky ukázaly, že je rozdíl mezi efektem SAIT trvající pouze dva roky a tříletou SAIT, přičemž delší léčba SAIT má větší efekt.

Abstract:

Approximately 25% of the European population suffers from pollen allergy, and for some patients who experience an exaggerated reaction that cannot be controlled by the regular use of antihistamines, allergen-specific immunotherapy (AIT) is used to help the immune system build a tolerance to the allergen. This bachelor's thesis focuses on the impact of AIT on the further use of pharmacotherapy, using the usual daily therapeutic doses (UDTD) for patients with pollen allergy. The objectives of the work include research on allergy and its treatment, identification of relevant records for the given question, and data analysis of the data set. Data for the investigation of the impact of immunotherapy come from the National Registry of Reimbursed Health Services, and statistical approaches such as paired and unpaired t-tests, Wilcoxon signed-rank test, and linear regression model were used for their analysis. The results of the analysis showed that AIT has an impact on further antihistamine consumption, in terms of reducing the necessary UDTD doses after treatment. Interestingly, the reduction in antihistamine consumption continues even after AIT is finished. Additionally, the results showed that there is a difference between the effect of AIT lasting only two years and a three-year AIT, with a longer AIT having a greater effect.
