Dependence of nursing care on arterial catheter functionality in the ICU

Abstract:

Úvod: Tato bakalářská práce se zabývá komplexní péčí o arteriální katetry na jednotkách intenzivní péče. Teoretická část shrnuje současně doporučované poznatky z této oblasti. Empirická část se zabývá problematikou předčasné extrakce arteriálních katetrů. Cíl práce: Hlavním cílem této bakalářské práce bylo zmapování problematiky předčasné extrakce arteriálních katetrů z důvodu malfunkce a vybraných aspektů, které mohou předčasnou extrakci determinovat na jednotce intenzivní péče (JIP) Kliniky anesteziologie, perioperační a intenzivní medicíny (KAPIM) v Masarykově nemocnici v Ústí nad Labem. Dílčími cíli dále: analyzovat současně doporučované trendy v péči o arteriální katetry, provést rozbor zdravotnické dokumentace vybraných pacientů s arteriálním katetrem, porovnat získané výsledky s dostupnými celosvětovými studiemi, vypracování sebehodnotící karty pro sestry v oblasti péče o arteriální katetry. Metodika: Retrospektivní analýza zdravotnické dokumentace vybraných pacientů. Použita byla kvantitativní metoda sběru dat průzkumného charakteru. Výsledky: V této práci jsme potvrdili výskyt problematiky předčasné extrakce arteriálních katetrů. Zmapovali jsme některé aspekty, které mohou vznik malfunkce ovlivnit. Byla navržena sebehodnotící karta pro sestry v intenzivní péči (IP). Závěr: Tato bakalářská práce se zabývala následujícími aspekty: kanylace arteriálních katetrů, ošetřovatelská péče o arteriální katetry, determinanty vzniku malfunkce, extrakce arteriálních katetrů, pokanylační komplikace, doporučení pro praxi v oblasti péče o arteriální vstupy. Nejzásadnějším řešeným problémem byly okolnosti extrakce arteriálních katetrů. Porovnáním se současnými světovými studiemi bylo zjištěno, že toto téma není doposud dostatečně prozkoumané.

Abstract:

Introduction: This bachelor thesis deals with complex care of arterial catheters in intensive care units. The theoretical part summarizes the currently recommended knowledge in this area. The empirical part deals with the issue of premature extraction of arterial catheters. Aim: The main objective of this bachelor thesis was to map the issue of premature extraction of arterial catheters due to malfunction and selected aspects that can determine premature extraction in the ICU KAPIM at Masaryk Hospital in Ústí nad Labem. Partial objectives are also to analyse currently recommended trends in arterial catheter care, to analyse the medical documentation of selected patients with arterial catheters, to compare the results obtained with available global studies, to develop a self-assessment card for nurses in the field of arterial catheter care. Methods: Retrospective analysis of medical documentation of selected patients. A quantitative method of data collection of a survey character was used. Results: In this work, we confirmed the occurrence of the problem of premature extraction of arterial catheters. We have mapped some aspects that can influence the occurrence of a malfunction. A self-assessment card for nurses in intensive care (IP) was designed. Conclusion: This bachelor's thesis dealt with the following aspects: cannulation of arterial catheters, nursing care of arterial catheters, determinants of malfunction, extraction of arterial catheters, post-cannulation complications, recommendations for practice in the field of arterial access care. The most fundamental problem solved was the circumstances of extraction of arterial catheters. By comparison with current world studies, it was found that this topic has not yet been sufficiently explored.
