Predictive factors of response to neoadjuvant chemotherapy in patients with breast cancer

Abstract:

V této diplomové práci se věnuji tématu léčby rakoviny se zaměřením na karcinom prsu, jež je nejčastějším zhoubným nádorem českých žen. Incidence má stoupající tendenci, a proto je snahou lékařských odborníků hledání nových, efektivnějších či nestandardních postupů při plánování protinádorové léčby. Tou by totiž měla být individualizovaná a maximálně efektivní terapie, která by byla postavena na základě genetického profilu nádoru a charakteristik pacientky. V souvislosti s tímto postupem je každý nádor klasifikován pomocí exprese tří hlavních markerů – receptorů pro estrogen, progesteron a HER2. Ty pomáhají při rozhodnutí, zda nasadit tzv. neoadjuvantní terapii před chirurgickým zákrokem. Cílem neoadjuvantní chemoterapie je zmenšení nádoru před plánovanou operací pro zmírnění radikality operačního zákroku. Při indikaci neoadjuvantní chemoterapie je nutné znát prediktivní markery odpovědi na cytostatickou léčbu, aby chemoterapie nebyla podána zbytečně. Některé markery jsou již standardně používány v klinické praxi, jiné jsou ve fázi výzkumů – například zvýšená exprese některých ABC transportérů v nádorové tkáni, které mohou modifikovat léčebnou odpověď na cytostatika. Ve své diplomové práci jsem pracovala se souborem 292 konsekutivních nádorů od pacientek z Masarykova onkologického ústavu v Brně s diagnózou karcinomu prsu, léčených neoadjuvantní chemoterapií, operací a eventuální adjuvantní terapií. Cílem práce bylo ze studie patologických a klinických záznamů jednotlivých pacientek stanovit sestavit databázi pacientek, obsahující co nejvíce různých faktorů, majících vliv na celkovou prognózu pacientek. Soubor byl dále upravován k následnému statistickému vyhodnocení, kdy byla zjišťována prediktivní hodnota jednotlivých markerů k odpovědi na neoadjuvantní cytostatickou léčbu a její dopad na celkové přežívání pacientek. Zvláštní pozornost byla věnována pacientkám s nestandardní odpovědí na neoadjuvantní chemoterapii, než jaká by byla předpokládána z doposud proběhlých výzkumů, neboť součástí mé diplomové práce bylo u těchto pacientek pomocí IHC stanovit hladiny exprese ABC transportérů P-gp a BCRP a pokusit se tak o něco více objasnit tuto složitou nádorovou problematiku. ABC transportéry P-gp a BCRP jsou totiž podle publikovaných výsledků spojovány s možnou chemorezistencí určitých druhů karcinomů prsu. Součástí mé diplomové práce bylo rovněž vytvoření příručky „Rakovina prsu v otázkách a odpovědích“. Tato příručka má sloužit (nejen) mladým ženám a dívkám k zodpovězení méně či více závažných otázek, souvisejících s touto problematikou. Otázky byly voleny podle toho, co nejvíce zajímalo ženy mezi 16 – 30 lety. Příručka by zároveň měla sloužit jako inspirace, návod a stimul k samovyšetřování a prevenci rakoviny prsu již u mladých žen a odtabuizování některých citlivých témat.

Abstract:

In this diploma thesis I deal with the topic of breast cancer treatment as it is the most common malignant tumor of Czech women. The incidence has an increasing tendency, and therefore are medical professionals seeking new ways to find more effective or non-standard procedures for anti-cancer treatment planning. This should be an individualized and thus maximally effective therapy that would be based on the genetic profile of the tumor and the characteristics of the patient. At this moment is this approach consisting of detection of three main markers – expression of receptor to Estrogen, Progesteron and HER2 in order to decide, whether use the so called neoadjuvant therapy before the surgical removal of the tumor. The goal of neoadjuvant chemotherapy is to reduce the tumor before planned operation to reduce the severity of the operation. When neoadjuvant chemotherapy is considered, it is necessary to know the predictive markers of cytostatic response to avoid unnecessary chemotherapy. Some markers are already used in clinical practice; others are in the research phase - for example, increased expression of some ABC transporters in tumor tissue that can modify the therapeutic response to cytotoxicity. I put together and worked with a set of 292 consecutive tumors from patients with a breast cancer diagnosis from the Masaryk Cancer Institute in Brno treated with neoadjuvant chemotherapy, surgery and alternatively with adjuvant therapy. The aim of the study was to put together vast variety of different factors influencing the overall prognosis of patients from the study of patients' pathological and clinical records. The file was further adapted to a subsequent statistical evaluation, where the predictive value of the individual markers was determined to neoadjuvant cytostatic treatment response and its impact on overall survival of the patients. Particular attention has been paid to patients with a non-standard response to neoadjuvant chemotherapy than would be expected from previously undergoing research as part of my thesis was also to immunohistochemicaly determine the expression levels of ABC transporters of P-gp and BCRP to try to clarify this complicated tumor problem a little more. ABC transporters P-gp and BCRP are reportedly associated with the possible chemoresistance of certain types of breast carcinoma. Part of my diploma thesis was also the creation of a book "Breast Cancer in Questions and Answers". This guide is intended to provide (not only) young women and girls with answers to issues related to this disease. The questions were chosen as the most interested ones for women between 16 and 30 years of age. At the same time, the handbook should serve as an inspiration, guide and stimulus to self-examination and prevention of breast cancer in young women and thus removing taboos of some sensitive topics.
