Physiotherapy in client with flatfoot and foot pressure changes after longtime insoles wearing

Abstract:

Úvod: Téma práce je Fyzioterapie u klienta s plochonožím a změny v tlacích chodidla do podložky po dlouhodobějším nošení vložek do bot. V této práci se zaměřuji na strukturu, vývoj a funkci klenby nohy a možnosti fyzioterapeutické intervence při léčbě plochonoží, s využitím nošení speciálně vyrobených vložek do bot. Cíl práce: Přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u pacienta s plochonožím. Metodika: Pacient s diagnózou plochonoží absolvoval 8 týdenní terapii, zároveň nosil 11 měsíců speciálně vyrobené D3D vložky do bot. Byl proveden vstupní a výstupní kineziologický rozbor před a po terapii, vstupní a výstupní měření přístrojem Footscan? v intervalu 11 měsíců. Výsledky: Zjištěné subjektivní a objektivní změny skončené terapie byly porovnány. Výsledky ukázaly změny ve snížení bolesti, zvýšení kloubní hybnosti, zvýšení svalové síly, zlepšení držení těla a stability. Porovnání výsledků měření na přístroji Footscan? prokázalo změny v zatížení chodidel. Vložky D3D nedokázaly zvýšit podélnou klenbu nohy. Závěr: Kombinace aktivního cvičení a nošení vložek do bot přispěla k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u pacienta s plochonožím.

Abstract:

Introduction: The title of the thesis is Physiotherapy in client with flatfoot and pressure changes after longtime insoles wearing. In this work I focus on the structure, development and function of foot arch and possibilities of active or passive physiotherapeutic intervention in flatfoot treatment, while wearing personalised D3D insoles. The aim of the work: A contribution to resolving the issue of selection of optimum physiotherapeutic procedures, concepts and methods on patients with flatfoot. Methodology: A patient with flatfoot underwent fifteen treatment sessions during a period of two months, while wearing personalised D3D insoles for eleven months. The therapy was focused on the muscles of lower limb and core exercise. Initial and final kinesiological analyses were performed after therapy. Changes in distribution of pressures and forces were compared after eleven months of insoles wearing. Results: The identified subjective and objective changes were compared before the therapy and after its completion. The results showed changes in the area of pain reduction, increase in joint mobility, increase in muscle-strength and in body-maintain and stability. Comparing the measured results on Footscan? showed changes in foot-load. The personalised D3D insoles failed to uplift the height of longitudinal arch. Conclusion: A combination of active exercise and longtime insoles wearing contributed to resolving of optimum physiotherapeutic procedures, concepts and methods in patients with flatfoot.
