Experiencing a High-risk Pregnancy

Abstract:

Cílem této práce bylo prozkoumat, jak přítomnost rizika či přímého ohrožení těhotenství ovlivňuje prožívání těhotné ženy. V teoretické části jsem definovala fyziologické, rizikové a patologické těhotenství a popsala jsem hlavní tělesné a psychické změny, kterými těhotné ženy procházejí. Dále jsem rozebrala zdravotnickou a sociální péči o těhotné ženy i ženy na rizikovém těhotenství. Podrobněji jsem se zabývala dosavadními poznatky psychologie těhotenství. Výzkum byl realizován pomocí kvalitativních rozhovorů s 15 ženami, které procházely rizikovým těhotenstvím stanoveným z objektivních medicínských důvodů. Přepsané rozhovory byly následně kvalitativně analyzovány. Z výsledků vyplývá, že prožívání žen na rizikovém těhotenství ovlivňuje mnoho faktorů, jako např. osobnostní dispozice, zatíženost porodnické anamnézy, očekávání vs. nečekanost vyskytnutých potíží, závažnost režimových opatření, míra sociální podpory, dostatečná informovanost a spokojenost se zdravotnickou péčí. Převládajícím pocitem je strach o dítě. Výrazný se ukázal i trend určité ambivalence v prožívaných pocitech. Rizikové těhotenství rovněž ovlivňuje navazování vztahu matka–plod. Čím vážnější je riziko v těhotenství, tím pravděpodobněji bude v postoji ženy přítomný určitý emoční odstup od dítěte, které čeká.

Abstract:

The major aim of this thesis has been to explore how the presence of a risk or a real threat to pregnancy influences the experiencing of a pregnant woman. In the theoretical part, I define the concepts of physiological, low-risk and high-risk pregnancy and describe the most important physical and mental changes which pregnant women undergo; I also discuss health and social care for pregnant women in general and women with high-risk pregnancy in particular. Special attention is paid to existing knowledge concerning the psychology of pregnancy. The research has been realised through qualitative interviews with 15 women undergoing a high-risk pregnancy diagnosed on the basis of objective medical assessment. The interviews have been transcribed and then qualitatively analysed. The results show that the experiencing of women with high-risk pregnancy is influenced by a number of factors, such as personal disposition, burdened obstetrical history, the (un)expectedness of problems, the character of a treatment precautions, the degree of social support, access to information and the degree of satisfaction with provided health care. The research suggests that the feeling that prevails are worries about the child’s life and health; generally, the women’s feelings are distinguished by certain ambiguity. High-risk pregnancy also influences the establishment of the relationship between a mother and her foetus: the higher the risk is, the more likely is an emotional distance in the attitude of a woman to her child.
