Abstract:

Cíl: Pro svou bakalářskou práci jsem si zvolila v dnešní době dost citlivé a diskutabilní téma a to Management bolesti při porodu. Chtěla jsem zjistit všeobecnou informovanost rodiček o metodách porodnické analgezie, zdroj jejich informací o této problematice a jejich postoj k jednotlivým metodám. Ve své práci jsem nejprve vysvětlila jednotlivé farmakologické i nefarmakologické metody v tlumení porodních bolestí. Značnou část práce jsem pak věnovala možnosti využití epidurální analgezie při porodu. Zaměřila jsem se na tuto metodu, dnes rodičkami velmi oblíbenou, abych zjistila, zdali jim přináší očekávaný komfort porodu bez bolesti. Metodika: Pro sběr empirických dat jsem použila nestandardizovaný dotazník, který byl celkově zaměřen na informovanost žen o porodnické analgezii. Pro lepší možnost srovnání jsem porovnávala skupinu respondentek z MNO (45 žen) a skupinu rodiček z MNO (43 žen). Výsledky: Většina žen přichází k porodu se základními vědomostmi o možnostech porodnické analgezie, tyto informace získávají hlavně z předporodních kurzů nebo internetu. Při porodu využívají také nefarmakologické metody, jako nejúčinnější však hodnotí epidurální analgezii. Při využití nedochází ve většině případů k negativnímu ovlivnění průběhu porodu, jako jsou slabé kontrakce a neschopnost tlačení při závěru II. doby porodní. Závěr: Porodnická analgezie a anestézie si v České republice během relativně krátké doby vydobyla pevné místo v porodnické praxi. Význam managementu bolesti v praxi se neustále zvyšuje. Nefarmakologické metody pomáhají rodičce se vyrovnat s porodními bolestmi ještě před vlastní aplikací léků, které při předčasném podání mohou bránit postupu porodu. Epidurální analgezie je dnes nejčastěji využívanou metodou v porodnictví, zaručující rodičkám dostatečný komfort v průběhu porodu.

Abstract:

Objective: For my bachelor work, I chose the childbirth pain management - pretty sensitive and debatable topic at the present time. I intended to ascertain the general awareness of women in childbed regarding methods of obstetric analgesia, sources of their information on these problems and their attitude toward particular methods. Within my work, I firstly explained single pharmacological as well as non-pharmacological methods of soothing labour pains. A great deal of the work I devoted to possibilities of using the epidural analgesia at childbirth. I focused on this method - today very popular among women in childbed - in order to find out whether the expected painless childbirth comfort is really achieved. Methodology: For collecting empiric data, I used a non-standardized questionnaire being generally aimed at womans awareness of the obstetric analgesia. For ensuring better reconciliation possibilities, I compared a group of respondents from MNO (45 women) and group of respondents from MNO (43 women). Results: Most of women come to childbirth with basic knowledge of obstetric analgesia possibilities whereas the information they mainly acquire in prenatal classes or from internet. In childbirth, the women also use non-pharmacological methods - however, they consider the epidural analgesia as the most efficient way. When applied, the negative affection of childbirth like weak contractions or inability to push towards the end of the second obstetric period does not occur in the majority of cases. Conclusion: In the Czech Republic, the obstetric analgesia and anaesthesia gained a firm position within the midwifery practice during relatively short time. The importance of practical pain management is increasingly growing. Non-pharmacological methods help women in childbed to cope with labour pains even before giving medications that, if dispensed prematurely, can impede the move of foetus. On the present, the epidural analgesia is the most frequently used method in obstetrics providing a sufficient comfort for women in childbed during labour.
