Diagnosis of Acute Kidney Injury in Children

Abstract:

Akutní poškození ledvin (AKI) je v současnosti jedním z nejdiskutovanějších problémů v dětské nefrologii. Onemocnění rychle progreduje a může přecházet až do chronického stádia. V současnosti se diagnostika AKI opírá o hodnoty kreatininu v krvi a z něho vypočítaný odhad glomerulární filtrace. Vzhledem k limitům stanovení kreatininu v krvi, není tento marker vhodný pro včasnou diagnostiku AKI. S tím se pojí zvýšené riziko přechodu onemocnění do chronického stádia při pozdním nástupu léčebné intervence, zvýšení rizika mortality, příp. dlouhý pobyt dětí na jednotkách intenzivní péče. Úspěšná léčba je spojena s časnou diagnostikou již ve fázi počínajícího rozvoje poškození. Z tohoto hlediska je stanovení nových biomarkerů v moči, především NGAL a KIM-1 významným přínosem v diagnostice AKI. Vzhledem k výše uvedeným aspektům se tato bakalářská práce zaměřuje na odhady referenčních intervalů NGAL a KIM-1 ve vzorku jednorázové moči. Tyto dva biomarkery byly stanovovány pomocí komerčních ELISA souprav na souboru pediatrických probandů bez renálního onemocnění. Naměřená data byla statisticky zpracována pomocí softwaru MedCalc a v souboru byla testována závislost na pohlaví a věku, která nebyla potvrzena. Biomarkery jsou vydávány formou indexu, tj. NGAL v poměru ke kreatininu v moči (NGAL/Kr), resp. KIM-1 v poměru ke kreatininu v moči (KIM-1/Kr) a jejich referenční rozmezí bylo vyhodnoceno u NGAL/Kr 0,55-8,24 ?g/mmol a u KIM-1/Kr 3.09-342.53 ng/mmol.

Abstract:

Acute kidney injury (AKI) is currently one of the most discussed problems in pediatric nephrology. The disease progresses rapidly and can progress to a chronic stage. At present, the diagnosis of AKI is based on blood creatinine values and an estimate of glomerular filtration calculated from it. Due to the limits of blood creatinine determination, this marker is not suitable for early diagnosis of AKI. This is associated with an increased risk of disease transition to the chronic stage at the late onset of treatment intervention, an increased risk of mortality, or. long stay of children in intensive care units. Successful treatment is associated with early diagnosis in the early stages of damage development. From this point of view, the determination of new biomarkers in urine, especially NGAL and KIM-1, is a significant benefit in the diagnosis of AKI. Due to the above aspects, this bachelor thesis focuses on estimates of reference intervals of NGAL and KIM-1 in a single urine sample. These two biomarkers were determined using commercial ELISA kits on a set of pediatric probands without renal disease. The measured data were statistically processed using MedCalc software and the set tested for dependence on gender and age, which was not confirmed. Biomarkers are issued in the form of an index, ie NGAL in relation to creatinine in urine (NGAL/Kr), resp. KIM-1 relative to urinary creatinine (KIM-1/Kr) and their reference range were evaluated for NGAL/Kr 0.55-8.24 ?g/mmol and for KIM-1/Kr 3.09-342.53 ng/mmol.
