Stochastics methods for evaluating of recurrent events in cardiology

Abstract:

Práce se zabývá aplikací statistických metod pro hodnocení výskytu rekurentních událostí, neboli událostí vyskytujících se opakovaně, v oblasti kardiologie. Opakovaná pozorování téhož pacienta mohou být vzájemně korelovaná a ovlivňovat výsledný model, což je vhodné zohlednit pomocí specifických přístupů. Spojením dat Národního registru kardiovaskulárních intervencí (NRKI) s registrem zemřelých získáváme pro analýzu objemný zdroj dat, který obsahuje záznamy o kardiovaskulárních intervencích provedených v České republice od roku 2005 s doplněnou informací o současném stavu pacienta. Pro determinaci rizikových faktorů a definici modelu pro predikci výskytu rekurentních událostí s úmrtím považovaným za informativní cenzorování byl použit joint frailty model pro rekurentní a terminální události. Práce dále obsahuje literární přehled publikovaných kardiovaskulárních predikčních modelů ve srovnání s nově navrženým modelem.

Abstract:

The thesis deals with statistical methods that can be used for the evaluation of the occurrence of recurrent events in cardiology. Specific methods have been proposed to handle the problem caused by possible correlation among repeated measures within patients. The National Register of Cardiovascular Interventions (NRKI) merged with the register of deaths creates the analysis-based data source which contains records of all percutaneous cardiovascular interventions performed in the Czech Republic since 2005 enriched with the current state of patients. Joint frailty model for recurrent events and a terminal event is used for the determination of risk factors and the prediction of reccurent events while considering the death as an informative censoring. The thesis includes a review of published cardiovascular risk prediction models which are compared to the newly designed model.
