Physiotherapy with the syndrome of painful shoulder

Abstract:

Cíl: Cílem bakalářské práce je přispět k řešení problematiky volby vhodných fyzioterapeutických metod, postupů a konceptů u pacienta se syndromem bolestivého ramene, konkrétně po sutuře m. supraspinati, a seznámit veřejnost s teoretickými i praktickými poznatky týkajícími se této problematiky. Metodika: V práci je zpracována kazuistika 70 leté ženy necelé 3 měsíce po sutuře musculi supraspinati v ambulantní péči. Pro objektivizaci byly využity metody měření, pozorování, vyšetření a rozhovoru pro získání anamnézy. Na základě vstupního kineziologického rozboru a speciálního vyšetření RK byl stanoven krátkodobý fyzioterapeutický plán. Terapie trvala 3 měsíce a pacientka absolvovala 16 terapií. V průběhu terapie bylo provedeno kontrolní vyšetření druhý, čtvrtý a osmý týden od začátku terapie. Byly využity techniky měkkých tkání (TMT), mobilizace, trakce, tlaková masáž jizvy, postizometrická relaxace (PIR), cvičení v otevřeném kinematickém řetězci (OKŘ) a uzavřeném kinematickém řetězci (UKŘ), cévní gymnastika (CG), dechová gymnastika (DG) a cvičení s pomůckami (overball, tyč, kuchařská rukavice, Propriomed). Koncepty proprioceptivní neuromuskulární facilitace (PNF), McKenzie mechanická diagnostika a terapie (MDT), Brüggerův koncept (cvičení s Thera-Bandem), koncept vzpěrných cvičení dle R. Brunkow, akrální koaktivační terapie (ACT), terapeutický koncept bazálních posturálních programů Jarmily Čápové nebo Spiraldynamik?. Pacientka byla poučena o správném stereotypu sedu a nutnosti korekce postury v běžných denních činnostech. Dále terapie byla doplněna cvičením v bazénu, které pacientka absolvovala 15 krát. Výsledky: V průběhu fyzioterapeutické intervence u pacientky došlo k výraznému zlepšení rozsahu pohybu, zvýšení svalové síly, snížení otoku a ke snížení svalové dysbalance, dále došlo ke zlepšení funkce celé horní končetiny. Závěr: Vlivem intervence a dobré spolupráce s pacientkou se stav značně zlepšil, díky tomu jsem si uvědomila, jak má velký vliv psychické ladění pacienta. Čím je zdravotní stav horší, tím je větší motivace k intenzivní spolupráci.

Abstract:

The aim: The aim of this Bachelory thesis si to contribute to resolve the problem of choosing the right physiotherapy methods, progresses and concepts at the patient with the syndrome of painfull shoulder, espacially after ruptura of m. supraspinati and show to public theoretical and practical knowledge. Methology: At this thesis we treated a patient 70 years old woman, whose diagnosis was after suture m. suprapinati in out patient care. To objectivise we used methods of measuring, observation, examination and discussion to the obtain the right diagnosis On the base of input kinesiological examination and special test of shoulder joint we have determined a short rehabilitation plan. The intervation lasted 3 months and consisted of 16 therapies. During these therapies was realised control examination second, fourth and eighth weeks after therapy started. There we used various methods: technics of soft tissues, mobilisatoin, traction, pressure massage of scar, post isometric relaxation, exercises in close and open kinetic chain, vascular gymnastic, respiratory gymnastic, exercises with aids (an overball, a stick, cooks gloves, Propriomed). Concept proprioceptive neuromuscular facilitation, McKenzie mechanical diagnostics and therapy, Brügger concept, concept of support exercises by R. Brunkow, acral coactivation therapy, concept of basal postural programs by J. Čápová or Spiraldynamic?. The patient was educated about right way of stereotype of sitting position and correction of posture in common daily routines. Very import part of the therapy were excercises in swimming pool, where the patient was 15 times. Results: During the course of physiotherapy movement the patient significally improved, there were increasing muscle strength, reducing edema and muscle disbalance. There was improving the complete function of arm. Conslusion: Thanks to intervention and good cooperation with the patient, her total condition has improved. Due to the cooperation I have realized how much important is good psychological condition. Worse health condition means better motivation for patient cooperation.
