Patient satisfaction survey in private medical network Santé

Abstract:

Diplomová práce se zabývá možnostmi aplikace sociologického výzkumu v prostředí monitorování spokojenosti pacientů ve zdravotnictví. V rámci této práce byla zpracována případová studie nestátního zdravotnického zařízení Santé, která charakterizuje a kriticky hodnotí stávající postupy průzkumů spokojenosti s cílem navrhnout metodologická zlepšení, která přispějí primárně ke zvýšení kvality takto získaných dat a sekundárně k efektivnějšímu řízení kvality poskytovaných služeb. V teoretické části jsou sumarizovány informace o různých typech sociologických šetření kvantitativního i kvalitativního charakteru, jejich metodologické základy, výhody a nevýhody, možná úskalí při implementaci. Zvláštní pozornost je věnována dotazníkovému šetření a focus groups. Teoretická základna je předpokladem pro vyhodnocení současné praxe nestátního zdravotnického zařízení Santé, které nyní využívá pro monitorování spokojenosti klientů standardizované dotazníkové šetření, webovou anketu a call centrum LINKU SANTÉ 1212. Jako nedostatky současného systému jsou identifikovány zejména úzké zaměření se na standardizovaný kvantitativní přístup ke sběru dat, metodologie výběru vzorku a nedostatečná reprezentativita jednotlivých šetření, některé nevhodně položené otázky v dotazníku a nízké využívání hodnocení prostřednictvím webové ankety. Možnosti zlepšení současného stavu jsou spatřovány v uplatnění techniky focus group, která umožní kromě získání hloubkových kvalitativních údajů i postižení skupinové interakce, a zavedení automatického telefonického dotazování prostřednictvím tzv. IVR, které s maximální úsporou nákladů umožňuje oslovit velké množství respondentů. Jedním z rozhodujících parametrů spokojenosti klientů je délka čekání na přijetí lékařem, kde je navrhováno využít interní informační systém společnosti Santé, tzv. WorkFlow, kde jsou k dispozici údaje, ze kterých je možné u každého klienta odvodit čekací dobu před vstupem do ordinace a tu pak dále zpracovávat podle odborností, lékařů, časového období, vývoje a reakce na přijatá nápravná opatření. Získané informace mohou být využity také k hodnocení jednotlivých lékařů v návaznosti na v Santé uplatňovanou variabilní složku mzdy. Výsledky jednotlivých šetření mohou výrazně přispět k lepšímu cílení poskytovaných služeb, jejich rozšiřování v souladu s požadavky klientů i ke zvýšení firemní kultury.

Abstract:

This thesis deals with the practical use of sociological research in patient satisfaction surveys in medical services. The case study of private medical network Santé is presented in this work. It describes and critically evaluates the current practice in satisfaction surveys in order to propose methodological improvements that will contribute primarily to improve the quality of obtained data, and secondarily to the more efficient management of the quality of health services. In the theoretical section the information on various types of sociological surveys of quantitative and qualitative nature, their methodological bases, advantages and disadvantages, potential difficulties in the implementation is summarized. Special attention is paid to the questionnaire survey and focus groups. The theoretical base is essential to evaluate the current practice of the private medical network Santé, which uses the standardized questionnaires, the web survey and the hot line LINKA SANTÉ 1212 for monitoring patients´ satisfaction. The shortcomings of the current system are mainly the narrow focus on the standardized quantitative approach to data collection, the sampling methodology and the lack of representativeness in particular surveys, some misleading questions in the questionnaire and the low use of evaluation through a web survey. The possibilities of improving the current situation are seen in the application of the focus group technique, which in addition to obtaining qualitative data in depth also provides the measuring of the group interaction, and the implementation of the automatic telephone interview, so called IVR, which allows reaching a large number of respondents with maximum cost savings. One of the crucial parameters of the customer satisfaction is the length of waiting for a doctor. It is proposed to use the internal information system of Santé, so called WorkFlow, where data, from which it is possible to derive the waiting period for each client, is available. This can be used to compare the waiting time according to the individual doctors, the medical specialization, the time period etc. The development and the response to the corrective actions can be also explored. The obtained information can be used to evaluate individual doctors and become the part of the variable wage components in the Santé rewarding system. The results of the surveys can significantly contribute to the better targeting of the provided services and their distribution in accordance to clients requirements and to enhance the corporate culture.
