Relationship of adherence and selected personality characteristics in patients with inflammatory bowel disease

Abstract:

Diplomová práce se zaměřuje na vztah mezi vybranými osobnostními charakteristikami a adherencí pacientů s idiopatickými střevními záněty. V teoretické části charakterizujeme Crohnovu chorobu a ulcerózní kolitidu, popisujeme adherenci a její vztah k vybraným osobnostním charakteristikám. Zaměřujeme se na self-efficacy, locus of control a Big five. Hlavním cílem empirické části bylo zmapovat adherenci pacientů s idiopatickými střevními záněty a prozkoumat vztahy mezi adherencí a vybranými osobnostními charakteristikami. Dalším cílem bylo zjistit, zda existují rozdíly v úrovni vybraných proměnných u pacientů ve stádiu relapsu a ve stádiu remise onemocnění. Do našeho výzkumu se zapojilo celkem 99 pacientů (47 s Crohnovou chorobou, 42 s ulcerózní kolitidou, 10 s nespecifikovaným typem nemoci), kteří v období prosinec 2018 až březen 2019 navštívili svého ošetřujícího lékaře v rámci gastroenterologické poradny. Zvolili jsme kvantitativní výzkumný design s využitím Visual Analogue Scale (VAS), Inflammatory Bowel Disease Self-Efficacy Scale (IBD-SES), Multidimensional Health Locus of Control Scale (MHLC) a Big five Inventory, zkrácená verze (BFI-10). Adherence byla pro účely zpracování dat rozdělena do třech dimenzí: léková adherence, adherence stravovacích a dietních opatření a celková adherence. Výsledky ukázaly, že míra adherence pozitivně koreluje s mírou self-efficacy a přívětivostí. Pozitivní vztah byl nalezen rovněž mezi adherencí, svědomitostí a powerful others, i když jen pro některé dimenze adherence. Bylo zjištěno, že pacienti ve fázi relapsu vykazují vyšší míru lékové adherence než pacienti ve fázi remise. Výsledky výzkumu ukázaly, že existuje vztah mezi osobnostními charakteristikami a adherencí pacientů s idiopatickými střevními záněty.

Abstract:

This diploma thesis is focused on the relationship of adherence and selected personality characteristics in patients with inflammatory bowel disease. In the theoretical part we characterize Crohn´s disease and ulcerative colitis. We describe adherence and its relation to selected personality characteristics. We focus on self-efficacy, locus of control and Big five. The main aim of the empirical part was to map the adherence of patients with inflammatory bowel disease and to explore the relationships between adherence and selected personality characteristics. Other aim was to find out whether there are differences in the level of selected variables in patients in the relapse and patients in the remission. Our sample consisted of 99 patients (47 with Crohn´s disease, 42 with ulcerative colitis, 10 with indeterminate colitis), who visited their attending physician in the gastroenterology clinic during December 2018 to March 2019. A quantitative design has been chosen. We used Visual Analogue Scale (VAS), Inflammatory Bowel Disease Self-Efficacy Scale (IBD-SES), Multidimensional Health Locus of Control Scale (MHLC) and Big five Inventory, short version (BFI-10). Adherence was divided into three dimensions: medication adherence, diet adherence, total adherence. The results showed that the level of adherence positively correlates with the level of self-efficacy and agreeableness. A positive relation has also been found between adherence, conscientiousness and powerful others, though only for some dimensions of adherence. Patiens in the relapse stage have been shown to exhibit a higher degree of medication adherence than patients in the remission stage. The results of the study indicated there is a substantial relationship between personal characteristics and adherence of patients with inflammatory bowel disease.
