Characterisation of Escherichia coli strains originated from extraintestinal infections

Abstract:

Escherichia coli je významným původcem extraintestinálních infekcí. Extraintestinální izoláty se od střevních odlišují např. ve výskytu faktorů virulence. V současnosti je velmi diskutována problematika rezistentních kmenů. U E. coli se nejčastěji vyskytuje produkce širokospektrých beta-laktamáz (ESBL). Celkem bylo srovnáváno 560 E. coli izolovaných z krve, moči a stolice. Močové izoláty se dělily do dvou skupin na základě schopnosti produkovat ESBL. U všech izolátů byl stanoven biochemický profil, schopnost hemolýzy, přítomnost antigenu O25 a citlivost k ciprofloxacinu, gentamicinu a ertapenemu. Dále byla u náhodně vybraných izolátů z různých tělních prostředí (29 z každé skupiny) testována schopnost tvorby biofilmu a vliv přídavku sacharidu do média nebo změny pH média na tvorbu biofilmu. Extraintestinální izoláty E. coli se od střevních escherichií lišily schopností metabolizovat některé substráty. Izoláty z krve byly častěji arginin-pozitivní. Extraintestinální izoláty méně využívaly melibiózu, rafinózu a sacharózu. Nejvíce rezistencí bylo zaznamenáno u ciprofloxacinu. Citlivost k ciprofloxacinu byla nejnižší u střevních (90,9 %) a nejvyšší u močových E. coli (96,7 %). Producenti ESBL byli častěji inositol-, ornithin-, sorbitol- a sacharóza-pozitivní. Významně méně metabolizovali arginin, dulcitol, eskulin a lysin. Další typickou vlastností producentů ESBL byla nízká schopnost hemolýzy (6,4 % oproti 34,7 % u non-ESBL). Mezi producenty ESBL byl častěji zastoupen i antigen O25 (42,5 % oproti 6,0 % u non-ESBL). Obohacení mozko-srdcové infuze (BHI) o sacharid a modifikace pH ovlivňovaly tvorbu biofilmu. Přídavek disacharidu do BHI produkci biofilmu podporoval, obohacení o monosacharid naopak tvorbu biofilmu, s výjimkou močových izolátů, většinou tlumilo. Při pH 5,0 docházelo ke zřetelnému poklesu produkce biofilmu. U producentů ESBL ale byla zaznamenána významně vyšší schopnost tvořit biofilm při pH 5,0 než u escherichií citlivých. Pro tvorbu biofilmu se u E. coli jako optimální jeví interval pH 6 až 6,5 a médium obohacené disacharidem. Extraintestinální izoláty z různých tělních prostředí se liší schopnostmi metabolizovat některé substráty, hemolyzovat, nebo tvořit biofilm v reakci na změnu vnějších podmínek.

Abstract:

Escherichia coli belong to important agents causing extraintestinal infections. The differences among extraintestinal and intestinal isolates are known, e.g. the virulence factors presence. Antimicrobial resistance is one of the most discussed current medical topics. The most common causes of E. coli resistance are Extended Spectrum beta-lactamases (ESBL) Producers. A total of 560 E. coli urine, blood and stool isolates were tested. E. coli obtained from urine were divided according to the ability to produce ESBL. Biochemical profile, haemolytic ability, the O25 antigen presence and susceptibility to ciprofloxacin, gentamicin and ertapenem were tested for each E. coli. The ability to form biofilm, the influence of saccharide addition to brain heart infusion (BHI), and the pH change were tested for randomly selected isolates (29 from each body environment) to establish the effect on biofilm formation. Extraintestinal E. coli differed from intestinal isolates in metabolic abilities. Isolates from blood cultures were more frequently arginine-positive. The extraintestinal isolates were less probable to metabolise melibiose, raffinose and sucrose. The highest number of resistant isolates was detected for ciprofloxacin. Its susceptibility was the lowest for stool (90.9%) and the highest for urine isolates (96.7%). ESBL producers were more frequently inositol-, ornithine-, sorbitol- and sucrose-positive. The positive reaction to arginine, dulcitol, esculin and lysine was less frequent in ESBL producers. The haemolysis on blood agar was less frequent in ESBL producers (6.4% compared to 34.7% for non-ESBL). Antigen O25 was typical for ESBL producers (42.5% compared to 6.0% for non-ESBL). Saccharide addition to BHI and pH modification influenced biofilm formation. Disaccharide addition to BHI increased biofilm formation, but monosaccharide addition usually decreased, with the exception of urine isolates, biofilm formation. The significant decrease of biofilm formation was detected at pH 5.0. However, ESBL producers were more likely to form biofilm under such conditions than the non-ESBL group. Biofilm formation in E. coli is enhanced with pH 6.0 to 6.5 and disacharide addition. Extraintestinal isolates from different body sites of origin differ in ability to metabolise substrates, to haemolyse or in ability to form biofilm under different external conditions.
