The concept of "Value" in the health sector: a systematic review

Abstract:

Tato práce zkoumá porozumění pojmu hodnota (angl. value) ve zdravotních systémech s využitím metod systematického přehledu literatury (systematic literature review). Cílem práce je zkoumat, jak se význam hodnoty ve zdravotnictví liší v existující litera-tuře, a tyto rozdíly pak vysvětlovat na kontextuální příslušnosti k různým typům zdra-votních systému a různým myšlenkovým proudům v oblasti HTA, zdravotnického ma-nagementu, public health a dalších věd. Do zkoumání bylo zařazeno více než 1700 zdro-jů identifikovaných pomocí vyhledávání v databázích PubMed a Scopus, ze kterých bylo následně vybráno 152 studií, které byly zkoumány v celém rozsahu. Zkoumání identifikovalo takřka 20 různých konceptů, které pracovaly s pojmem hodnota. Pri-mární rozdíl mezi koncepty spočíval v evropském a americkém přístupu k hodnotě. Další rozdíly v porozumění hodnoty ve zdravotnictví vycházely ze šíře zaměření, kde se definice lišila mezi mikro přístupy, které zkoumaly hodnotu konkrétního léku nebo hodnotu z pohledu pacienta, a makro přístupy, které se snažily hodnotu definovat na úrovni systému. Tyto dva aspekty byly použity k vytvoření klasifikační matice, umož-ňující čtenáři zdravotnické literatury porozumět kontextu konkrétní akademické práce podle zařazení do této matice. V závěru práce je diskutován význam jednotlivých iden-tifikovaných konceptů ve zdravotnictví České republiky.

Abstract:

This work aims to investigate the meaning of value in the context of the healthcare system utilizing systematic review methodology. The goal of this work is to uncover variations in the meaning of “value” within the existing healthcare literature based on association with different national healthcare systems as well as different scientific disciplines such as HTA, healthcare management or public health. The methodology of this work included retrieving more than 1700 entries from PubMed and Scopus databases, which were further narrowed down to 152, which were studied in full length. The process identified almost 20 different concepts that related (to larger or smaller degree) to the meaning of value. The primary difference lied in the distinction between European and American understanding. Additional differences stemmed from the scope of definition, where micro-value would be used in connection to an evaluation of a particular medicine, method or device, whereas macro-value related to concepts attempting to define value on the level of the system. These two differences in meaning where used to create a classification matrix allowing the reader to understand the context of a particular piece of work. The final chapter of this work was dedicated to consideration of applicability of selected concepts into the Czech healthcare system.
