Availability of outpatient physiotherapy in the Ústí Region - juridical adjustment vs. reality and perspective vs. real needs

Abstract:

Abstrakt Úvod: V úvodu diplomové práce se nejprve věnuji vysvětlení výběru tématu a motivací k problematice dostupnosti ambulantní fyzioterapie. Následuje přehled teoretických poznatků, kde je na základě analýzy veřejně dostupných dat provedena charakteristika regionu Ústecký kraj a jeho obyvatelstva ve vybraných parametrech, včetně srovnání vybraných parametrů s ostatními kraji České republiky. V další části práce nabízí teoretická východiska o obecném uspořádání a základních principech zdravotnického systému Cíl práce: Cílem diplomové práce je zkoumat zkušenosti obyvatel, indikujících lékařů i samotných poskytovatelů s aktuální reálnou dostupností ambulantní fyzioterapie v Ústeckém kraji. Dále, s cílem přispět k řešení problematiky dostupnosti zdravotní péče v odbornosti ambulantní fyzioterapie, se pokusit analyzovat a pojmenovat možné příčinné faktory zhoršující dostupnost ambulantní fyzioterapie a navrhnout doporučení pro praxi, směřující ke zlepšení situace. Metodika: Na základě cílů práce byly formulované předpoklady vyjádřeny v šesti výzkumných otázkách a k nim stanoveny konkrétní hypotézy. Statistickou metodou výzkumu, bylo kvantitativní šetření s využitím formy dotazníkového průzkumu. Třemi cílovými skupinami pro dotazování byli obyvatelé, poskytovatelé ambulantní fyzioterapie a indikující lékaři z Ústeckého kraje. Data získaná pomocí celkem tří nestandardizovaných dotazníků, směřovaných ke třem cílovým skupinám dotazování, byla využita k prověření výzkumných otázek, statistickému testování konkrétních hypotéz a k diskutování aktuální situace ohledně problematiky dostupnosti ambulantní fyzioterapie v Ústeckém kraji. Výsledky: Hlavním zjištěním průzkumu je převážně negativním obecné hodnocení dostupnosti ambulantní fyzioterapie, na němž se shodují všechny tři statistické jednotky. Převážně negativní názor vyjádřili jak dotazovaní obyvatelé (v 65,7%), tak poskytovatelé (v 71,2%), i lékaři (v 71,9%). Respondenti průzkumu, se shodují i na tom, že několikatýdenní čekací doby u poskytovatelů zhoršují přístup pacientů k péči a oddalují reálné zahájení léčby. Celkem se průzkumu zúčastnilo 493 respondentů. Závěr: Řešení pro zlepšení dostupnosti ambulantní fyzioterapie nespočívá jen v dalším navyšování kapacity sítě poskytovatelů této péče. I v současném nastavení systému lze jistě hledat způsoby, jak každá ze zúčastněných stran může změnou vlastního přístupu významně přispět ke zlepšení stávající situace. Abstract:

Summary Introduction: The diploma thesis introduction includes reasons, why the theme of the work was chosen and why the motivation for dealing with ambulant physiotherapy accessibility problems arised. It also contains a theoretical knowledge summary, within the characteristic of the Ústecký Region and its inhabitants is made. Special selected parameters of the region are compared with other regions in the Czech Republic. The next part of the thesis introduces theoretical bases of the Czech medical system general organization and its basic principles Goal: This diploma thesis goal is to research experiences of residents, indicating doctors and physiotherapy providers with the current access to physiotherapy in Ústecký Region. Next goal is to analyse and name possible causal factors of the failing ambulant physiotherapy access with the intention of solving medical physiotherapy care problems. And of course to come up with practice suggestions aiming for improvment. Methodology: The diploma thesis formulates assumtions that are put into six research questions and there exists determined hypothesis for each of them. A guantitative inquiry with using questionnaire was chosen as a statistical research method. Three goal quistionnaire groups were inhabitants, providers of the ambulant physiotherapy and indicating doctors from the Ústecký Region. Data from three unstandardized questionnaire given to three goal groups were used to answer research questions, to test the hypotheses and to discuss the current situation and problems of the ambulant physiotherapy care access in the Ústecký Region. Results: The main results of the research is mostly negative evaluation of the ambulant physiotherapy care access, all three statistical units concured. Mostly negative opinion was showed by the inhabitants (65,7 %), the providers (71,2 %) and even by doctors (71,9 %). The research responders concured that many weeks wainting time given by physiotherapy providers makes care access much worse and postpones the real care institution. A total of 493 respondents took part in the survey Conclusion: The solution for a better ambulant physiotherapy access doesn't lie in a providers capacity increase. Even in the current system it is possible to find ways how every involved side can contribute to the improvement of this situation with its own attitude change.
