Comparison the effectiveness of myoptic and hyperopic LASIK 1999-2010

Abstract:

Cíl: Autorka hodnotí efektivitu a bezpečnost chirurgické metody LASIK, referuje o výsledcích korekce refrakční vady u pacientů s mírnou, střední a těžkou myopií v dlouhodobém časovém horizontu. Podrobně je rozebrána i problematika indukovaného astigmatismu. Samostatnou kapitolu tvoří shrnutí výsledků korekce mírné, střední a těžké hypermetropie, kde výsledky nejsou validní, a proto se LASIK u hypermetropie již neužívá. Cílem všech hodnocených zákroků byla vždy emetropie. Metodika: Retrospektivní srovnávací studie Výsledky: Autorka hodnotí pooperační výsledky chirurgické metody LASIK. V rámci myopie porovnává soubor B z roku 2007 - 2010 ošetřený excimer laserem Zyoptix® TECHNOLAS Z 100 se souborem A z roku 1999 - 2005 ošetřený excimer laserem Keracor 117 Chiron Technolas. Autorka ukazuje na snížení rozptylu výsledků o 40-50%, dále pak jsou u souboru B prakticky eliminovány extrémní výsledky, což svědčí o přesnější prediktabilitě pooperačních výsledků. V případě nekorigované zrakové ostrosti (UCVA), nejlépe korigované zrakové ostrosti (BCVA), indexu efektivity a bezpečnosti lze pozorovat u obou souborů obdobné střední hodnoty, avšak znatelně větší variabilitu pooperačních výsledků u souboru A. Rovněž tak indukovaný astigmatismus je u souboru B 3x menší než u souboru A. Výsledná hodnota astigmatismu je u souboru B prakticky poloviční než u souboru A. Autorka poukazuje i na nepřesnou aplikaci výpočtů indukovaného astigmatismu, kterou zaznamenala při studiu literatury k této oblasti. Vzhledem k neuspokojivým pooperačním výsledkům u pacientů s hypermetropií z let 1999 - 2005, se již v posledních letech nepřistupuje k řešení této vady metodou LASIK. Závěr: Na svých souborech autorka představuje a porovnává dlouhodobé výsledky LASIKu, které potvrzují vysokou efektivitu, bezpečnost i prediktabilitu této chirurgické metody v rámci myopie do hodnot -10 dioptrií a omezené použití LASIKu v rámci hypermetropie.

Abstract:

Aim: The author evaluates the effectiveness and the safety of LASIK surgery method and reports on long-term results of correcting refractive errors by LASIK in patients suffering from low, middle and high myopia. Similarly, she analyses the issue of induced astigmatism. A special chapter is formed by summarising results of low, middle and high hyperopia correction, which results are not valid, and therefore LASIK for hyperopia has not used more. The aim of all the evaluated surgeries has always been emetropia. Method: Retrospective comparative study Results: The author has had experience in LASIK surgery method since 1996 and evaluates the post-operative outcome of her own work consecutively in two refractive centres, both equipped with sophisticated instrumentation, in the period given. Concerning myopia, she compares Group B of years 2007 - 2010 using excimer laser Zyoptix® TECHNOLAS Z 100 to Group A of 1999 - 2005 using Keracor 117 Chiron Technolas. She points out a 40-50%, decrease in the results variance for Group B, and further on, she finds out that in Group B extreme results are virtually eliminated, which gives evidence of a more precise predictability of postoperative results. In the case of Uncorrected Visual Acuity (UCVA), Best Corrected Visual Acuity (BCVA) and the effectiveness and safety indexes, similar mean values can be observed in both groups, but there is a considerably higher variance of postoperative results in Group A. Likewise, induced astigmatism is finally three times lower in Group B than in Group A. The author also points out an inaccurate application of induced astigmatism calculations which she came across when studying literature related to the issue. Due to unsatisfactory postoperative outcome in patients with hyperopia from the years 1999 - 2005, hasn´t been approach to resolving of this refractive errors by LASIK procedure in recent years. Conclusion: On her groups of patients the author presents and compares long-term LASIK results which confirm high effectiveness, safety and predictability of this refractive surgical procedure in treating myopia up to 10 dioptres, and a limited usage of LASIK in treating hyperopia.
