Support of peristalsis in patients after radical cystectomy in intensive care

Abstract:

Cílem diplomové práce bylo posoudit efekt zavedených opatření na podporu peristaltiky u pacientů po radikální cystektomii. Dílčími cíli bylo zhodnotit efekt nefarmakologických a farmakologických opatření, vliv léčby bolesti a vliv rizikových faktorů na obnovu peristaltiky u těchto pacientů. Výzkumný soubor tvořilo 48 pacientů hospitalizovaných na Klinice anesteziologie, resuscitace a intenzivní medicíny Fakultní nemocnice Ostrava v období leden 2019 až prosinec 2020. Data byla získána retrospektivní analýzou zdravotnické dokumentace a následně byla statisticky zpracována. Vztahy mezi jednotlivými parametry byly hodnoceny pomocí Chi-kvadrátového testu a Fischerova exaktního testu. Pro hodnocení vlivu léků byla použita jednofaktorová neparametrická ANOVA a dvouvýběrový t-test s nulovou hypotézou. Statistické testy byly hodnoceny na hladině významnosti 5 %. Z výsledků šetření vyplynulo, že existuje statisticky významný rozdíl mezi pohlavím a BMI pacienta a mezi obnovou peristaltiky. Statisticky významný rozdíl nebyl potvrzen mezi obnovou peristaltiky a časnou vertikalizací, kinesiotapingem břicha, farmakologickými opatřeními, časně zahájenou enterální výživou, vlivem epidurální analgezie, vlivem opioidních analgetik, věkem a délkou hospitalizace. Na základě zjištěných skutečností a po podrobném zkoumání problematiky, lze konstatovat, že je daná problematika aktuální. Ošetřovatelský personál představuje důležitou část multidisciplinárního týmu, který se společně s lékaři, fyzioterapeuty a dalšími členy podílí na péči o pacienty.

Abstract:

The aim of the master´s thesis was to assess the effect of established measures to support peristalsis in patients after radical cystectomy. The partial objectives were to evaluate the effect of non-pharmacological and pharmacological measures, the effect of pain treatment and the influence of risk factors on the recovery of peristalsis in these patients. The sample group consisted of 48 patients hospitalized at the Department of Anaesthesiology, Resuscitation and Intensive Care, University Hospital Ostrava, in the period from January 2019 to December 2020. Data were obtained by retrospective analysis of medical records and subsequently were statistically processed. The relations between the individual parameters were evaluated using the Chi-square test and the Fischer exact test. One-factor nonparametric ANOVA and a two-sample t-test with a null hypothesis were used to evaluate the effect of drugs. Statistical tests were evaluated at a significance level of 5%. The results showed that there was a statistically significant difference between the patient's gender and BMI and between the restoration of peristalsis. Early verticalization, abdominal kinesiotaping, pharmacological measures, early enteral nutrition, epidural analgesia, opioid analgesics, age and length of hospital stay did not confirm as statistically significant for recovery of peristalsis. Based on the findings and a detailed examination of the issue, it can be stated that the issue is current. Nursing staff is an important part of a multidisciplinary team that works with doctors, physiotherapists and other members to care for patients.
