Abstract:

Lékové exantémy představují závažný zdravotní problém pro vysoký výskyt a stále stoupající incidenci v důsledku vývoje nových léků a léčebných schémat, rozsáhlé i neuvážené preskripci léčiv. Jejich diagnostika je obtížná a komplikovaná pro omezené možnosti průkazu klinickými nebo laboratorními testy. Nemáme k dispozici specifický konfirmační test s 100% senzitivitou a specifitou. Při zjišťování kauzálního léku vycházíme z anamnézy, klinického obrazu a průběhu onemocnění. Cílem práce bylo zhodnotit soubor pacientů s lékovým exantémem léčených za hospitalizace na I. dermatovenerologické klinice v letech 2005-2009 a u vybraných pacientů ověřit několik diagnostických možností pro potvrzení léku jako příčinného agens. Náš soubor zahrnoval 433 pacientů a odpovídal 11,4 % ze všech hospitalizací. Byla patrná převaha žen (303, 70 %, s průměrným věkem 48,7±19,2 roku) oproti mužům (130, 30 %, s průměrným věkem 50±18,1 roku). V etiologii exantémů se nejčastěji uplatnila antibiotika (199x) s nejčastějším zástupcem amoxicilinem (109x). Na druhém místě byla analgetika (108x). V klinickém obraze dominoval makulopapulózní exantém. S různými typy exantémů se ve 30 % případů současně vyskytly i další příznaky jako febrilie, lokalizovaný otok kůže kdekoliv na těle nebo sliznici, celková slabost, nauzea nebo vomitus. Pacienty s lékovým exantémem s předpokládaným vznikem na podkladě pozdního typu přecitlivělosti jsme vyšetřili epikutánními testy (ET). ET jsou považovány za standard v diagnostice lékové přecitlivělosti pozdního typu. Jejich senzitivita se podle literárních údajů pohybuje v rozmezí 7,5–86,7 % v závislosti na klinickém obrazu a léku. U těchto pacientů byla vyšetřena také hladina celkového IgE a u vybraných léků i specifické IgE. ET jsme vyšetřili celkem 111 pacientů (77 žen a 34 mužů ve věku od 17 do 84 let s průměrným věkem 52,5±16,6 roku). Pozitivní výsledky byly zaznamenány u 5 ze 111 případů (tj. v 4,5 %). Hladina celkového IgE byla zvýšená u 27 % pacientů. Specifické IgE bylo vyšetřeno u 75 ze 111 pacientů, s hraničním výsledkem u 2 pacientů (tj. u 2,7 %). U vybraných 8 pacientů (ze 111; s lékovým exantémem po tribenosidu) bylo provedeno podrobné komplexní vyšetření. Kromě epikutánních testů byly doplněny speciální laboratorní diagnostické metody (test proliferace lymfocytů a test aktivace bazofilů). Pozitivní reakce v epikutánních testech byla zaznamenána u jednoho pacienta. Test proliferace lymfocytů vyvolal jeden hraničně pozitivní výsledek u jiného pacienta a test aktivace bazofilů byl pozitivní u dalšího pacienta. Výsledky provedených komplexních funkčních vyšetření u našich pacientů ukazují možnost rozvoje IV. i I. typu imunopatologické reakce v patogenezi exantému vyvolaný tribenosidem. Alergologické a imunologické vyšetření společně s epikutánními testy považujeme za hodnotné a nezbytné příspěvky v diagnostice lékové alergie, které mohou ozřejmit patogenezi lékových reakcí. Zvyšují senzitivitu a specifitu diagnostického procesu. Pro rutinní použití těchto metod bude nutné dále propracovat metodiku přípravy a aplikace antigenu a lépe poznat imunologické mechanismy vedoucí ke vzniku hypersenzitivity u jednotlivých léků. K tomu měla přispět i naše práce.

Abstract:

Cutaneous adverse drug reactions (CADR) represent a severe health problem. High prevalence and incidence of CADR are still increasing due to the development of new drugs and treatment schemes, extensive and injudicious prescription of drugs. The diagnosis is difficult and complicated because of the limited possibilities for verification by clinical or laboratory tests. We do not have specific confirmatory test with 100% sensitivity and specificity. The history of CADR, clinical course and picture are essential for the determination of the type of immunological reaction and the appropriate choice of diagnostic tests. The aim of the study was to evaluate a group of inpatients treated in 1st Department of Dermatovenereology for drug eruption in 2005-2009 and in selected patients to verify several options for diagnostic confirmation of the drug as a causal agent. Our cohort consisted of 433 patients and corresponded to 11.4 % of all hospital admissions. There was apparent predominance of women (303, 70 %, with an average age of 48.7 ± 19.2 years) than men (130, 30 %, with a mean age of 50 ± 18.1 years). The causative agents of CADR were most commonly antibiotics (199x) with the most frequent amoxicillin (109x). Analgesics (108x) were on the second place. Maculopapular exanthemas dominated in the clinical picture. Other symptoms such as fever, localized swelling of the skin anywhere on the body or mucous membranes, weakness, nausea or vomiting occurred at the same time with various types of exanthemas in 30 % of cases. Patients with drug eruptions presumably on the basis of delayed type hypersensitivity were investigated by patch tests (PT). PT are considered standard in the diagnosis of delayed type of drug hypersensitivity. Their sensitivity, according to literature data, ranges from 7.5 to 86.7 % depending on the clinical picture and drug. Levels of total IgE and specific IgE of selected drugs were also examined in these patients. We examined a total of 111 patients (77 women and 34 men aged 17-84 years with a mean age of 52.5 ± 16.6 years) by patch tests. Positive results were found in 5 out of 111 cases (i.e. 4.5 %). Level of total IgE was elevated in 27 % of patients. Specific IgE to drug was tested in 75 of 111 patients. Borderline result was found in 2 patients (2.7 %). Selected 8 patients (of the 111 patients, with a CADR induced by tribenoside) were examined by a detailed complex cohort of immunological tests. Patch tests were performed to investigate in vivo cellular reaction. Laboratory investigations were fulfilled with lymphocyte transformation tests and basophil activation tests. We found a positive patch test reaction to tribenoside in one patient. Lymphocyte transformation test elicited one border positive reaction in one patient and the basophil activation test gave a clearly positive reaction in another patient, respectively. The results of the complex functional tests in our patients prove the possibility of developing delayed and immediate type of immunopathogenic response in the etiology of tribenoside induced exanthemas. We consider allergological and immunological examinations together with patch tests as valuable and necessary essential contributions in the diagnostics of CADR that can elucidate the pathogenesis of drug eruptions. In vitro immunological tests and patch tests increase the sensitivity and specificity of the diagnostic process. The methodology of preparation and application of antigen will be further elaborate for routine use of these methods and better understand the immunological mechanisms leading to hypersensitivity of individual drugs. This should also contribute to our work.
