Abstract:

Úvod: Současné pojetí konceptu ERAS zahrnuje inovace v oblasti perioperační péče. V teoretické části disertační práce bylo cílem popsat dle protokolu ERAS specifický přístup k pacientům ve fázi předoperační, operační a pooperační. V praktické části disertační práce bylo cílem vyhodnotit, stupeň implementace jednotlivých dílčích postupů z protokolu ERAS ve vybraných zdravotnických zařízeních a zdali zavádění těchto dílčích postupů do perioperační péče vede k rychlejší rekonvalescenci po operačních výkonech a ke zkrácení doby hospitalizace u chirurgických pacientů. Metodika a charakteristika souboru: Data byla získávána na základě stanovených kritérií ze zdravotnické dokumentace vybrané skupiny pacientů po plánovaných resekcích střeva, s pobytem na jednotce intenzivní péče (JIP) minimálně 48 hodin. Výzkumný soubor byl sestaven z pacientů hospitalizovaných na dvou chirurgických JIP. Ve zdravotnickém zařízení v Karviné (JIP 1) bylo do studie zahrnuto 79 pacientů a v Havířově (JIP 2) bylo zařazeno 107 pacientů. Cílem retrospektivní studie bylo srovnání pooperační péče s vybranými elementy protokolu ERAS (Enhanced Recovery After Surgery): optimální analgetizace, neagresivní tekutinová náhrada, prevence nevolnosti a zvracení, časný perorální nebo enterální příjem, časné odstraňování drénů, nasogastrických sond a permanentních močových katétrů, časná rehabilitace. Výsledky: Na základě retrospektivní analýzy získaných informací je patrné, že konkrétní dílčí postupy z protokolu ERAS byly zaváděny a realizovány na obou pracovištích JIP. Statisticky významný rozdíl byl zjištěn u optimální pooperační analgetizace (epidurální katétr) a dodržení tekutinové restrikce do 2,5 litrů. Dále byly zaznamenány statisticky významné rozdíly u četnosti provedených laparoskopií, u časného perorálního a enterálního příjmu, u časného odstraňování drénů, nasogastrických sond a permanentních močových katétrů. Závěr: Na sledovaných pracovištích se míra implementace ERAS v jednotlivých krocích liší, ale v uvedené studii nevedly tyto rozdíly k rozdílným výsledkům. Prokazatelně se nedá určit, který element z koncepce ERAS má signifikantní vliv na konečné výsledky.

Abstract:

Introduction: The current concept of the ERAS concept includes innovations in the field of perioperative care. In the theoretical part of the dissertation, the aim was to describe a specific approach to patients in the preoperative, operative and postoperative phases according to the ERAS protocol. In the practical part of the dissertation, the aim was to evaluate the degree of implementation of individual partial procedures from the ERAS protocol in selected medical facilities and whether the introducing these partial procedures into perioperative care leads to faster recovery after surgery and shortening hospital stay in surgical patients. Methodology and characteristics of the group: Data were obtained on the basis of established criteria from the medical documentation of a selected group of patients after planned intestinal resections, with a stay in the intensive care unit (ICU) of at least 48 hours. The research group was composed of patients hospitalized in two surgical ICUs. Seventy-nine patients were included in the study at the medical facility in Karviná (ICU 1), and one hundred and seven patients were enrolled in Havířov (ICU 2). The aim of the retrospective study was a comparison of postoperative care with selected elements of the ERAS (Enhanced Recovery After Surgery) protocol: optimal analgesia, non-aggressive fluid replacement, nausea and vomiting prevention, early oral or enteral intake, early removal of drains, nasogastric tubes and permanent urinary catheters, early rehabilitation. Results: Based on a retrospective analysis of the information obtained, it is evident that specific partial procedures from the ERAS protocol were implemented at both ICU workplaces. A statistically significant difference was found in the optimal postoperative analgesia (epidural catheter) and adherence to fluid restriction up to 2,5 liters. Furthermore, statistically significant differences in the frequency of laparoscopies, early oral and enteral intake, early removal of drains, nasogastric tubes and permanent urinary catheters were noted. Conclusion: The level of ERAS implementation in the monitored workplaces differs in individual steps, but in the mentioned study these differences did not lead to different results. It is demonstrably impossible to determine which element of the ERAS concept has a significant effect on the final results.
