Determination of tyrosine kinase inhibitors by LC MS for therapeutic monitoring their plasma concentrations

Abstract:

Sunitinib je perorálně podávaný multikinázový inhibitor receptorových tyrozinkináz standardně využívaný pro léčbu metastatického adenokarcinomu ledviny a imatinib-rezistentního gastrointestinálního stromálního tumoru. Z důvodu velké variability ve farmakokinetickém profilu mezi pacienty je nutné provádět terapeutické monitorování léčiv, a to především kvůli vyššímu riziku nežádoucích účinků a toxicity při vysokých plazmatických koncentracích, a naopak neúčinnosti terapie u příliš nízkých koncentrací. K tomuto účelu je standardně využívána metoda vysokoúčinné kapalinové chromatografie s tandemovou hmotnostní spektrometrií. Při stanovení sunitinibu a jeho primárního metabolitu N-desethylsunitinibu v plazmě je zapotřebí překonat problém geometrické izomerace obou sloučenin, kdy je nutné buď během celé analýzy chránit vzorky před světlem, nebo provádět sumaci signálů jednotlivých izomerů. Výše zmíněné postupy nejsou příliš vhodné pro klinicko-farmaceutickou praxi, proto bylo cílem mé diplomové práce ověření a zavedení metody zpětné konverze izomerů, která by tyto problémy překonala.

Abstract:

Sunitinib is a peroral administered multi-targeted receptor tyrosine kinase inhibitor commonly used to treat metastatic renal adenocarcinoma and imatinib-resistant gastrointestinal stromal tumor. Due to the large variability in the pharmacokinetic profile between patients, therapeutical drug monitoring is necessary, mainly because of the higher risk of adverse effects and toxicity at plasma high levels and, reversely, the inefficiency of the therapy at too low plasma levels. For this purpose, the method of high-performance liquid chromatography with tandem mass spectrometry is used as the standard. When determining sunitinib and its primary metabolite N-desethyl sunitinib in plasma, it is necessary to overcome the problem of geometric isomerization of both compounds, where it was necessary to either protect the sample from light or sum the signals of individual isomers throughout the analysis. For this purpose, the method of reconversion of isomers has been developed which overcomes these problems in the determination. The procedures mentioned above are not very suitable for clinical-pharmaceutical practice, so the aim of the thesis was to introduce and implement this new method for laboratory use.
