Identification and functional characterization of small non-coding RNAs in urine as biomarkers of urologic cancers

Abstract:

Karcinom močového měchýře je nejčastěji se vyskytující malignitou vylučovacího systému s celosvětově narůstající incidencí a mortalitou. Stejně jako u ostatních onkologických diagnóz je pro prognózu pacientů klíčový především záchyt onemocnění v jeho počátečních stádiích. Limitace současných diagnostických přístupů, mezi které patří právě nízká senzitivita pro časná stádia onemocnění a invazivní povaha vyšetření, vedou ke snaze identifikovat nové neinvazivní biomarkery, které by umožnily citlivou detekci onemocnění. Velký potenciál má detekce cirkulujících krátkých nekódujících RNA v moči, jelikož expresní profily těchto molekul mohou vzhledem k jejich významné roli v patogenezi přímo odrážet stav onemocnění. V této práci byly s využitím metody sekvenování nové generace analyzovány globální profily mikroRNA a PIWI-interagujících RNA v moči pacientů s karcinomem močového měchýře a zdravých kontrol. Vybrané nekódující RNA, které byly u pacientů s tímto onemocněním deregulovány, byly dále validovány na nezávislé kohortě 60 subjektů. Významně zvýšená hladina v moči pacientů byla potvrzena u miR-93-5p (p <0,0001) a let-7i-5p (p <0,0001). Hladina piR-1044 byla u pacientů signifikantně snížená (p = 0,043). Na buněčných liniích odvozených od karcinomu močového měchýře byla poté ověřena aktivní sekrece a mezibuněčný přenos miR-93-5p. Výsledky této práce naznačují potenciální využití nekódujících RNA v moči k neinvazivní detekci karcinomu močového měchýře.

Abstract:

Bladder cancer is the most common cancer of urinary tract with an increasing incidence and mortality worldwide. As with other cancer diagnoses, detection of the disease at its early stages is crucial for patient prognosis. Limitations of current diagnostic methods, such as low sensitivity for early stages of the disease and invasive nature of the procedure, have led to a focus on search for non-invasive diagnostic biomarkers, which would allow the detection of the disease with high degree of sensitivity and specificity. Detection of short non-coding RNAs in urine holds a great potential. Given their role in pathogenesis of the disease, their extracellular expression profiles could reflect the disease status. In this study, next-generation sequencing was used to determine global profiles of microRNAs and PIWI-interacting RNAs in urine of bladder cancer patients and healthy donors. Selected noncoding RNAs, which were deregulated in patients with bladder cancer, were further validated in an independent cohort of 60 subjects. Significantly upregulated levels of miR-93-5p (p <0,0001) and let-7i-5p (0,0001) in patient samples were confirmed. PiR-1044 was significantly downregulated (p = 0,043) in patient samples. Using cell lines derived from urothelial bladder carcinoma we demonstrated active secretion and intracellular transfer of miR-93-5p. Results of this study indicate potential use of noncoding RNAs in urine for non-invasive detection of bladder cancer.
