Use of the Balance Tutor device in the rehabilitation of the LCA of the knee joint in athletes

Abstract:

ABSTRAKT Úvod: V mé bakalářské práci jsem se zabývala využitím přístroje BalanceTutor v rehabilitaci LCA kolenního kloubu a problematikou fyzioterapie po ruptuře LCA kolenního kloubu u sportovců. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky LCA u sportovců a poukázat na doplňkové využití BalanceTutoru při komplexní rehabilitaci LCA. Metodika: Hlavní část práce je věnována kazuistice 23leté pacientky po plastice předního zkříženého vazu pravého kolenního kloubu. Nejprve jsem vypracovala vstupní kineziologický rozbor. Součástí vstupního kineziologického rozboru byla anamnéza pacienta, antropometrie, goniometrie, svalový test dle prof. Jandy (2004), specifické testy pro kolenní kloub a testy hluboké stabilizace trupu. Na základě vstupního vyšetření byl probandovi stanoven krátkodobý fyzioterapeutický plán, který byl složen z 12 terapií. Terapie probíhaly 1x týdně 90 minut po dobu 3 měsíců. V průběhu krátkodobého rehabilitačního plánu bylo po 6 týdnech provedeno kontrolní vyšetření se zaměřením na měkké tkáně, svalovou sílu, obvody dolních končetin a rozsah pohybu pravého kolenního kloubu do extenze. Po 3 měsících byl v rámci poslední terapie vypracován výstupní kineziologický rozbor. Na základě výsledků výstupního kineziologického rozboru byl zpracován dlouhodobý fyzioterapeutický plán. Výsledky: S pacientkou se nám podařilo obnovit fyziologický rozsah pohybu do flexe v pravém kolenním kloubu. Rozsah pohybu do extenze v pravém kolenním kloubu zatím stále není plný, ale došlo ke zlepšení. Podařilo se zvýšit svalovou sílu, při pohybu do extenze a zlepšila se také posturální trupová stabilizace. Pacientka pociťuje větší stabilitu kolenního kloubu a větší jistotu při pohybu. Závěr: Práce s pacientkou mě bavila a obohatila mě o nové zkušenosti. Doufám, že touto prací přispěji k řešení problematiky poranění LCA a k rozšíření informací o přístroji BalanceTutor.

Abstract:

SUMMARY Introduction: This bachelor thesis deals with the use of the BalanceTutor device in the rehabilitation of the ACL knee joint and the issue of physiotherapy after ACL knee joint rupture in athletes. The objective: The aim of this thesis is to contribute to solving the issue of LCA injuries in athletes and to also highlight the complementary use of the BalanceTutor device within the complex rehabilitation procedure of ACL. Methodology: The main part of the thesis focuses on a case study conducted on a 23-year-old female patient who underwent an anterior cruciate ligament surgical operation of the right knee joint. First, I conducted an initial kinesiological analysis including patient's history, anthropometry, goniometry, muscle test according to Prof. Janda (2004), I run specific tests for the knee joint as well as deep stabilization tests of the body. Based on the initial examination, the proband was presented with a short-term physiotherapy plan consisting of 12 sessions. The therapies were conducted once a week for 90 ninutes for 3 months. During the duration of the short-term physiotherapy rehabilitation plan, a follow-up examination was performed afteer 6 weeks, focusing on soft tissue, muscle strength, lower legs circumference, and range of motion of the right knee joint into extension. After 3 months, an outcome kinesiological analysis was performed as part of the last therapy session. Based on the results of the outcome analysis, a long-term physiotherapy rehabilitation plan was devised. Results: We were able to restore the physiological range of motion to flexion in the right knee joint with the proband. The range of motion into extension with the right knee is still not up to its full capacity, but there was a gradual improvement after the sessions. Muscle strength has increased during extension and postural trunk stabilisation has improved. The proband feels stability in the joint has increased and thus feels more confident while moving it. Conclusions: Working with the patient has been enriching to me. I hope that this thesis will contribute to solving this issue of ACL injuries and will also help to spread information on using the BalanceTutor device.
